Blood after levothyroxine treatment

Hello

I just received my bloods after three months on levothyroxine - 25 micrograms once a day.

TSH: 0.005 T3: 6 T4 13.3

Background: Hashimoto

Should I be on a higher dose? It seems my T3/T4 are in range but TSH suggests I’m hyper..

My previous bloods before treatment were:

First: TSH 0.005 (0.39-5.33), T4 16.3 (7-16), T3 7.3 (3.8-6)

Second: TSH 9.36, T4 6.3

Third: TSH 14.1 T4 7.5

Your results suggest you have Hashimoto’s also called autoimmune thyroid disease

Hashimoto’s frequently starts with transient hyperthyroid results and symptoms before becoming increasingly hypothyroid

Essential to regularly retest vitamin D, folate, ferritin and B12

What vitamin supplements are you currently taking

Have you had coeliac blood test

My gp has just called and said they want me to take levothyroxine 25mcg on alternate days now because of my bloods. They refused to take vit b12

I have asked for coeliac test

Your levels are swinging up and down almost certainly because of Hashimoto’s/autoimmune thyroid disease

Strongly recommend getting full thyroid and vitamin testing done privately in New year

Strictly gluten free diet often helps or is essential

For full Thyroid evaluation you need TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested. Also EXTREMELY important to test vitamin D, folate, ferritin and B12

Low vitamin levels are extremely common, especially if you have autoimmune thyroid disease (Hashimoto's) diagnosed by raised Thyroid antibodies

Ask GP to test vitamin levels and thyroid antibodies

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water .

Last dose of Levothyroxine 24 hours prior to blood test. (taking delayed dose immediately after blood draw).

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Is this how you do your tests?

Private tests are available as NHS currently rarely tests Ft3 or thyroid antibodies or all relevant vitamins

If TPO or TG thyroid antibodies are high this is usually due to Hashimoto’s (commonly known in UK as autoimmune thyroid disease).

About 90% of all primary hypothyroidism in Uk is due to Hashimoto’s. Low vitamin levels are particularly common with Hashimoto’s.

Gluten intolerance is often a hidden issue to.

The aim of levothyroxine is to increase dose upwards until Ft4 is in top third of range and Ft3 at least half way through range (regardless of how low TSH is) ...important to have optimal vitamin levels too as this helps reduce symptoms and improve how levothyroxine works
