Does anyone know Uk address for Blue Horizon? I posted a blood test and foolishly forgot to enclose a form, I have emailed BH but am still waiting for a reply. Thanks

The sample doesn't go back to Blue Horizon, it goes to a lab - often County Pathology, sometimes The Doctor's Laboratory. Do you know which lab name/address was on the return envelope?

Sorry don't know - didn't really look!! I just feel such an idiot forgetting form, I have used BH many times so should know better. Apart from anything else I feel I may have completely spent money that I cant afford for nothing...Woe is me

The test tubes normally have bar codes on with your details....(well Medichecks do) ...so should be oK

Obviously you need to contact them

Have tried contacting via email but have had no reply (expect they are busy now). Don't know phone no. or I would try that!

SlowDragon I've just checked my BH kit, there is no bar code on the tube, there is a printed label with all your details on and that is attached to the tube.

Did you send it back Special Delivery, if so then there will be a post code on the receipt. I have a County Pathology return envelope here and I can let you know if it's them if you say what the post code is.

I know you haven't sent the form back, I'm asking did you send the sample back Special Delivery? I think the form may be a standard form that goes to whichever laboratory they use.

I have sent the sample back on the container they supplied. It just said to post in any royal mail box which I did

But postal service during pandemic is extremely slow. It’s been recommended to post all test kits back tracked special delivery

If it arrives late and is unusable they should send another

Thanks for your replies, keeping fingers crossed it's just a hold up in postal services. The thing is I have ordered a few things on line and no delay whatever. Still everywhere has it's own way and I think BH is a small concern

Their website gives all their latest information. They have withdrawn their phone and online chat services. This is their latest advice on their Contact Us page:

Was it just a normal standard first class return envelope or have they temporarily upgraded it to "Tracked24" like Medichecks, which gets it there within 24-48 hours. If it was standard first class then delivery is being delayed and from personal experience I have had both post I have sent and received by first class taking up to a week, so I hope your sample reaches the lab before it spoils.

Normal standard . Contacted via email as per webpage advice but I think maybe replies there also delayed.Will just have to wait and see!

They aim to reply with 2 working days to the contact form.

Did you stick the label onto the tube? It has your name, date of birth, gender, date and time sample was taken? That will at least identify you.

The Form that says "Laboratory Request Form Source" is to identify which company (or doctor) has requested the test and who has to pay for the lab's services. As your email address isn't on that form then I imagine results are sent to Blue Horizon who then send them to you.

Whichever lab does the test also does them for other companies, eg Medichecks, Thriva, plus others. So the lab needs to know who has requested the test in order to send the results to the right company. I have looked at everything in my kit and nothing other than the form you missed out identifies the company as being Blue Horizon from what I can see.

Maybe Paul from BlueHorizonMedicals will see that I've tagged him and may be able to help.

Thanks again, I have just received a reply from BH and they are concerned and have given me a phone contact to use if I don't here from the lab within 24 hours Perhaps not all lost!
