Hey All,

I have been on 112 mcg Levo and 17.5 mcg t3, these are my results:

Tsh = 0.06 (0.27-4.2)

Ft4 = 16.8 (12-22) 48%

Ft3 = 5.7 (3.1-6.8) 70%

When I was on 125 mcg Levo and 10 mcg t3 I was overheating, low grade fever 37.5, hot flushes, was not able to raise my t3 dose past 10, tried 3 times.

My Endo says I should go up to 20 mcg t3 and drop t4 to 100 mcg at first, he thinks Levo gives me symptoms. I'm still fatigued, swollen and I have issues falling asleep, I'm overweight and struggle with my mood a bit, my pulse is normal or lowish. He thinks I might end up on 50-75 mcg t4 and 20-40 mcg t3. Maybe he is right not sure. I know some people have to have their ft4 low to feel good.

Any advice would be appreciated. All I know is that I cannot raise Levo to 125, it was too much and it was giving me jittees, it was high in range.

Looking through previous posts couldn’t see any vitamin results

What are your most recent vitamin D, folate, B12 and ferritin levels

Do you have hashimoto’s?

Are you on strictly gluten free diet?

Hi SlowDragon, by b12 was over range recently, vit d is over 100, ferritin only 35% of range , but I've been supplementing for a long time, iron is 75% range, folate is mid range.

I have Hashimotos, I have been gluten free for years, didn't do anything for me.

Hey SlowDragon actually me and my partner thought of something. I should try to increase Levo by 12.5 with ft4 being under mid-range. Since lowering, I don't feel quite myself, I'm cold and still underactive even though I have upped my t3 from 10 to 20. Have you heard of stories of people who were able to increase or re-introduce Levo after a while even though before was not possible, even before taking a bigger dose of t3? I think I've read something like this before.my tsh has jumped to 0.06 from under 0.01 on a lower dose before, which suggests something too, not that I care for tsh at all :)

Many of us find we need good levels of Ft4 and ft3

Trying small dose increase in levothyroxine might improve symptoms

Thanks SlowDragon appreciate it, I will try it on in a couple of days..my Endo is so focused on t3 only , he's not a big fan of t4

Well I was as high as 150 t4. Reduced down to 137, then 125 when I felt kinda the best, but still hypo, ft4 was around 75% on 125 with 10 mcg t3 at 36%. Reduced to 112, ft4 plummeted to 48% from 75% over 7 weeks later, that's with 17.5cg t3 at 70%. Now I'm at 20 mcg t3 for a few days, but I'm cold, low temps etc.

Frequently endocrinologists seem to reduce levothyroxine too much

Absolutely. He wanted me to go as low as 50/75, I'm pretty sure that won't be good for me. Cognitively I'm better on more Levo. I was just wondering if sometimes we need to go back to a dose that before we were not able to tolerate with so much t3. I'd give it a try :) nothing to lose.

My own experience

On 125mcg ....unable to tolerate higher dose.

Eventually T3 is prescribed

Levothyroxine reduced to 100mcg

20mcg T3 added

FT4 dropped lower and lower.....and symptoms returning ....

After 4 months on 100mcg ...I increased levothyroxine to 112mcg. Tested after 8 weeks. Ft4 stopped dropping but not rising. Slight improvement in symptoms

Increased levothyroxine back to 125mcg. Ft4 recovered and symptoms resolved

Hi SlowDragon forgot to ask, if you don't mind. My ft3 is 70% on 17.5 t3 and 112 t4. The last few days I have been taking 20 + 112, I will be upping my Levo to 125, which will bring my ft3 high over range I suspect, should I reduce back to 17.5 or even 15? Thank you x

We usually recommend only increasing one or the other..... if you increase levothyroxine...retest in 6-8 weeks

You could then consider upping T3 ...depending on results obviously

Thank you. I may then stay on 20 mcg t3 and 112 t4 for a few weeks. When did you notice improvements SlowDragon ? I have been on 15 mcg for 4 weeks, 3 weeks on 17.5 and only a few days on 20. My bloods above were on 17.5... I may be rushing things ?

I took it very slow

Initially Levo was reduced from 125 to 112. I added 5mcg T3 (as 2 x 2.5mcg ...speck of dust) instant noticeable and significant improvement

After 3-4 days, reduced Levo to 100mcg and upped T3 to 2 x 5mcg.

Held at this for 8 weeks. I always feel worse 4-7 weeks after any dose change. So mix of better and worse.

Blood tested.

Then T3 upped to 20mcg (10mcg, 5mcg, 5mcg) and no levothyroxine reduction

After 4 months on reduced Levo, Ft4 had dropped from 20 (12-22) to 14

Cautiously increased levothyroxine to 112mcg

Retested after 8 weeks

Ft4 stationary at 14

Increased to 125mcg. Ft4 slowly improved and slight increase in Ft3

SlowDragon this is helpful, I was curious about your timelines! I actually do feel worse last month, a good day, then a really bad day and again. Glad to hear this is normal! My ft4 is still mid range so I'm holding that and I'm holding my 20 of t3 for the next weeks, even though it can be terrible at times, especially the sleep.

I find taking 5mcg T3 at bedtime improves sleep

Many people find the same ....but not everyone

Discovered by accident when I moved my 6pm dose to later in evening to leave the correct 8-12 hour gap between last dose T3 and blood test

I used to take T3 at 6am, noon and 6pm

For last 2 years taken T3 at 7am, 3pm and 11pm

(10mcg, 5mcg and 5mcg

I also split my levothyroxine....shouldn’t make a difference...but seems to. 100mcg at 11pm and 25mcg at 7am

Always same brands of Levothyroxine and T3. Can’t tolerate changing brands

Magnesium moved to early evening (minimum four hours before Levo and T3)

All vitamins optimal and absolutely strictly gluten and soya free

Yes I ALWAYS feel grim about 4-7 weeks after any dose change. I think it’s rest of endocrine system trying to rebalance....adrenals especially

Thank you SlowDragon, you're so kind giving me all this info! Yes, a crumb of t3 gives me a good sleep.

I tried to take 125 mcg of t4 again yesterday and I was already, on a fist day, overheating and had anxiety and restlessness. So today I continue with my 112 and 20 t3. I've read on that some members here also need their t4 lower to feel better. Sooo many options potentially

I know.....it took me about 2 years experimenting with doses....splitting....not splitting..different timings ...

Some people can manage single dose T3, others twice daily. I couldn’t tolerate either

For me 3 doses at eight hour interval seems smoothest.

Well has to be done! Thankfully, I tolerate all the t3 in one dose now. Now it's just a bit of a waiting game and then either upping or lowering t4.

Hi SlowDragon last question I promise :) my ft3 was at 70% on 17.5 t3. I have been taking increased t3 dose of 20 mcg t3 the last few days and yesterday I started to feel anxious, uneasy, palpitations out of a sudden very brief. My blood test was done 24 hrs after last dose of t3 so I assume my t3 can be high now.

I take it all at once, works the best for me . I have tried splitting weeks ago and also a year ago, it made me feel worse and more hypo. I took the same dose today to see how I get on. If too much I either drop to 17.5 and hold or drop Levo to 100 from 112 and up my t3 further. It's all so complicated, but I think I'm getting closer and closer

Hey SlowDragon me again :) something weird is happening. I added 20 mcg to my 112 mcg and I feel even more hypo, how could it be? Haven't seen many cases like this in the forum. I think maybe I should drop back to 17.5 and start adding Levo back. I can't think of anything else, dropping levo further to 100 seems counterproductive if I feel so hypo. My pulse is slow, anxiety back, horrible itchy skin, dry nose etc etc :) and after many days of good sleep, now sleeping badly. I guess my only worry is that many times I tried to add more than t3 to 125 of Levo, next dose up, and I ended up with fast pulse and palps.

It’s all about experimenting with what works for you

Only make one change at a time. Either levothyroxine or T3

Ideally waiting 6-8 weeks to retest bloods

Thanks, I want to change Levo to 125, but I can't with t3 at 20, I'll be overmedicated, I need to drop it I'm afraid.

I just don't understand how I become more hypo on adding t3, would love to know :(
