Hypo to Hyper??

Hi folks,

I’ve been feeling unwell recently so went back to get my thyroid bloods done, I have been Hypothyroid now for 5 years. I am taking 100mcg of Levothyroxine and 10mcg T3 (Liothyronine).

My FT4 came back at 16.2

My TSH is 0.1

Does this now mean I’ve gone from Hypo to Hyper?

As you are on t3 the most important result is the Ft3 result which you have not got. Without that the others tell us nothing.

For full Thyroid evaluation you need TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested. Also extremely important to regularly retest vitamin D, folate, ferritin and B12

Low vitamin levels are extremely common, especially if Thyroid antibodies are raised

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water . This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip, best not mentioned to GP or phlebotomist)

Last Levothyroxine dose should be 24 hours prior to test, (taking delayed dose immediately after blood draw).

If/when also on T3, make sure to take last dose 8-12 hours prior to test, even if this means adjusting time or splitting of dose day before test

Is this how you do your tests?

Private tests are available. Thousands on here forced to do this as NHS often refuses to test FT3 or antibodies or all vitamins

Medichecks Thyroid plus ultra vitamin or Blue Horizon Thyroid plus eleven are the most popular choice. DIY finger prick test or option to pay extra for private blood draw. Both companies often have special offers, Medichecks usually have offers on Thursdays, Blue Horizon its more random

Most important results are FT3 and FT4

TSH is almost always suppressed on any dose of T3.

Just to add it isn't possible to go from hypo to hyper as such, but it is possible to be over medicated. As you are taking T3 the TSH isn't always much use. Without the range for the free T4 measurement it is hard to say for sure, but 16 is not usually a high value. As others have said, you also need to know your free T3 amount to get a full picture.
