hypothyroid grandson?

hi i posted here some months ago about my grandsons thyroid readings. his TSH had reached 9 and docs agreed to leave it till next blood test in 2 months. in the meantime, i bought pure high strength liquid b vits from a thyroid site in america. at his next blood test his tsh reading was 4.5 which was much better and he was feeling better. he had an appointment to see an endocrinologist and i went with him. the doc said he was not displaying other typical symptoms i.e. not feeling cold, tired and his weight gain was because he had stopped playing football. he said he would not consider treating with levothyroxine until his tsh was at least 10! (this is new to me as it used to be 5 before). in any event, to give doc his due he has tested him for adrenal problems i.e cortisol testing and also for testoterone levels. i also said he had not had a T3 reading done so he has ordered that as well and also a vitD reading. does anyone have any advice as to what could be wrong if it is not his thryroid but something else giving him a high tsh reading. i am confused since seeing the endocrinologist

The only other possibility is a pituitary problem. The pituitary produces TSH, so it could be making too much. This assumes he has no typical hypothyroid symptoms at all, and his freeT4 and freeT3 blood tests would not come back low.
