Sorry to post again but I feel like I need some further advice as I feel like I am going mad and not sure which way to go next..!
So I have asked to be referred which has been agreed in the meantime I had some bloods done which are as follows
AB peroxidase 35
it doesn’t give me what the reference range is for this but I am assuming it’s within range as it came back no further action.. my antibodies were 85 on last testing so does this now mean I don’t have autoimmune thyroid as it’s come down?
Tsh 0.02 (0.27-4.2)
T3 4.9 (2.8-7.1)
T4 25.8 (9-26)
B12 423 (191-663)
Folate 4.4 (3.3-19.3)
I also take 2x500 metformin for pcos
Currently take 150 Levo
Completely do not feel right I feel like I get both hypo and hyper symptoms and still don’t really feel any better
Cold extremities
Bloating
Tired
Aches, pins and needles
Racing heart
Palps
Air hunger
Nightmares
Anxiety
Internal shaking
A mix of loose stools and constipation
Sore breasts and pain in left breast
Lack of periods or very lengthy cycles ( I am also on the mini pill this is my second month and I was hoping that would help with some of my pms symptoms)
I can’t loose any weight despite going to slimming world and just feel completely like I’m having a melt down as I can’t figure oi wvat is wrong!
Do you feel I am now over medicated based on these results?
Should I ask for a reduction in Levo?
Could this be related to cortisol or adrenal issues?
Any advice would be appreciated

You need BOTH TPO and TG antibodies tested plus vitamin D and ferritin
Your folate level is very low as well
For full Thyroid evaluation you need TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested. Also extremely important to test vitamin D, folate, ferritin and B12
Low vitamin levels are extremely common, especially if Thyroid antibodies are raised
Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and fasting. Last Levothyroxine dose should be 24 hours prior to test, (taking delayed dose immediately after blood draw). This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip, best not mentioned to GP or phlebotomist)
Is this how you do your tests?
Private tests are available. Thousands on here forced to do this as NHS often refuses to test FT3 or antibodies or all vitamins
Medichecks Thyroid plus ultra vitamin or Blue Horizon Thyroid plus eleven are the most popular choice. DIY finger prick test or option to pay extra for private blood draw. Both companies often have special offers, Medichecks usually have offers on Thursdays, Blue Horizon its more random
Hashimoto's affects the gut and leads to low stomach acid and then low vitamin levels
Low vitamin levels affect Thyroid hormone working
Poor gut function can lead leaky gut (literally holes in gut wall) this can cause food intolerances. Most common by far is gluten. Dairy is second most common.
According to Izabella Wentz the Thyroid Pharmacist approx 5% with Hashimoto's are coeliac, but over 80% find gluten free diet helps, sometimes significantly. Either due to direct gluten intolerance (no test available) or due to leaky gut and gluten causing molecular mimicry (see Amy Myers link)
Changing to a strictly gluten free diet may help reduce symptoms, help gut heal and slowly lower TPO antibodies
Ideally ask GP for coeliac blood test first or buy test online for under £20, just to rule it out first
Trying gluten free diet for 3-6 months. If no noticeable improvement then reintroduce gluten and see if symptoms get worse
Yes all tests are always at least 24hrs and first thing on fasting
They didn’t test vitamin d but have done previously and it was fine back in Jan my ferritin was above range slightly too the last time they took it so I definitely don’t think this is an issue
I do supplement vitamin d and also now folate too which also has b12 in it my b12 is also quite high in range
I’ve had a coeliac test done and it was negative
I’m looking to self medicate with t3 or if that doesn’t help then ndt to see if this improves the way I feel
I’ve spoke to a clinic in London who will do a private prescription for both so I’m going to get this organised asap
B vitamins best taken in the morning after breakfast
Recommended brands on here are Igennus Super B complex. (Often only need one tablet per day, not two). Or Jarrow B-right
If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before any blood tests, as biotin can falsely affect test results
Calm vitality magnesium powder is popular as it's cheap and easy to use. Good for helping improve constipation. Best to start on small dose as too much can cause diarrhoea. Helps with sleep so best taken in afternoon or evening (must be four hours away from taking any thyroid hormones)
Better You make magnesium flakes, great for bath or foot bath. Very relaxing and skin feels wonderfully soft after
Selenium.
vega yeast free selenium Or Solgar Yeast free vitamin E with selenium.
(candida is common hypothyroid problem, so avoiding yeast may be good idea)
There is a slight suspicion that you might be heading towards poor conversion as FT4 is highish in the range and FT3 lower. This should improve if you get your vits and minerals optimal. I would t change medication for two reasons-when addressing things you may not need it but going over to NDT or adding T3 can cause problems in some if conversion hasn't been addressed properly.
I had a conversion issues, corrected it and went on NDT. T4 was certingly letting me down plus I'd started many years earlier on NDT and preferred it so went back and had no problems switching over.
