I have been feeling pretty rubbish lately so thought I had better check my levels. I currently take 100mcg Levo and 18.75mcg T3. I stupidly stopped taking my supplements before Xmas as I was feeling quite well so I know I need to start them again.
Please can you give me some advice, thanks.

Your TSH levels look excellent, and everything else seems in range. perhaps the T3 could be higher but I'm no expert. I find when I feel rough it's all too easy to blame the thyroidism, but it's not always the case. How's your blood glucose levels?
Are you sure you have read the report correctly? The FT4 is below range and the FT3 is very low in range. The Vit D level is also below range, and the folate and ferritin low in range.
I think you know you have Hashi's. Are you gluten free and supplementing with selenium l-selenomethionine to try and reduce the antibodies? Gluten free works for some people but there's no guarantee.
When we take a combination of Levo and T3, we each have our own individual place where our FT4 and FT3 need to be. Personally, I need both about 75% through range. Others do well with a much lower FT4 and FT3 towards the upper end of the range. Do you have any previous results where you felt well so that you know what levels to aim for?
Of course, having Hashi's causes fluctuations in results and symptoms so that complicates things.
Are you still having B12 injections?
You need a B complex and that should raise your folate level. It shold be at least half way.
Your Vit D should be around the level recommended by the Vit D Council - 125nmol/L - and the Vit D Society - 100-150nmol/L - and you need D3's important cofactors magnesium and Vit K2-MK7.
You also need to get your ferritin up to half way through range - eating liver, liver pate and black pudding can help.
Thanks Seaside Susie! your advice is always excellent. Yes I am due a B12 tomorrow, my last results I started to feel a bit better TSH 0.73 (0.27-4.2), T3 4.52 (3.1-6.8), FT4 15.00 (12-22). I was on the same dose of Levo but only 12.5mcg T3 which I increased to 18.75 T3. I am now wondering if I should go back to my original dose of 125mcg Levo alongside the 18.75mcg T3 or increase the T3 to 25mcg. I suppose it's trial and error.
I did an awful lot of tweaking to find the right combination for me. It might be worth increasing Levo for now, that will increase FT4 and may increase your FT3 as well, depending on how much natural conversion you do have.
I actually went up to 31.25mcg T3 at one point, but am now settled on 18.75mcg along with my Levo.
