Hi again. My dose of NDT was raised 2 weeks ago from 11/2 to 2 grains as was undermedicated. I’m now getting palpitations and flushing should I have gone up a 1/4 grain rather than a half?

I see from this post healthunlocked.com/thyroidu... which unfortunately went unanswered (probably a busy time on the forum and it disappeared off the main page quickly), that you have raised TG antibodies and you suspect Hashi's. In that case it could possibly be antibody fluctuation that's causing the palps and flushing, in which case a slight lowering of your NDT may help anyway, or you only needed a 1/4 grain increase at this time. Slow and steady increases are always best anyway, so that you don't miss your sweet spot.

Ok I’ll reduce by a 1/4 grain tomorrow. My antibodies on last test were below range managed to get them down. Thank you again

You may have reduced your antibodies - although that's difficult to prove because they fluctuate all the time - but you still have Hashi's. The antibodies are not the disease. :)

Antibodies come in after an attack by the immune system - i.e. lymphocytes invade the thyroid, and cause damage, releasing thyroid peroxidase and/or thyroglobulin into the blood stream - where they shouldn't be. So, the thyroid peroxidase and/or thyroglobulin antibodies then come along and envelope them and carry them away to dispose of them. They clean up the scene of the crime, as it were.

But, you don't have to have high antibodies to have Hashi's. There are people who never have high antibodies, and their Hashi's is diagnosed by an ultrasound.

Would you drop 1/4 grain for 2 weeks then increase 1/4 grain again or leave it longer?

I would reduce by 1/4 grain, see how you feel after 2 weeks. That dose may suit you. Go by how you feel, especially if your antibodies are fluctuating. No point in raising dose if your thyoid hormone levels are still high. If you start to get symptomatic again, possibly when hormone levels start to fall after antibody activity, you can try raising by 1/4 grain and see how that is.

When you get up to the 2 grain mark it's best to increase more gradually otherwise it's easy to overshoot x

I followed instruction to increase in half grains up to 2 then proceed slowly by quarters. My sweet spot was 1.75 so had already missed it! I got up to 3 grains and have testing and saw Endo! FT3 reading was 6 and top of range 5.7! I new I was t right but still felt ok so I thought the drop was too much but he was spot on!

Ah ok I felt under on 1.5 and I’m definitely over on 2!
