I just got last weeks test results back. I am currently on 10mcg T3 and 50mcg T4.
Still feeling lethargic at times and sore do I add more T3 or more T4? I didn’t do RT3 as already have been on T3 for past 5 months.

It's always a juggling act to try and find the right balance of Levo and T3 and it takes a lot of tweaking of doses. It depends on where you need your FT4 and FT3 levels to feel best.
Personally, and I take both Levo and T3, I need FT4 to be at around 75%, I don't feel well with it low in range. I also need FT3 around 75%, but everyone is different.
Your TSH is still a bit high for a treated hypo patient, especially considering that you're taking T3.
Maybe you need both FT4 and FT3 higher in range. But don't change both doses at the same time.
If you increase your Levo, depending on how much natural conversion you have that may increase your FT3. So maybe it's worth trying a rise in Levo dose first, then retest in 6-8 weeks (you wont need rT3 testing).
Thank you for looking Susie. Do my levels look lower in the ranges? What upped dose of T4 is a good place to start? I think you are right both ultimately will need to go up but I agree starting with T4 initially.

Do you mean lower than your last results?
Last results I can see from your previous posts are
TSH 0.98mU/l range 0.2-4.5 mU/L
T4 14pmol/l range 9-21 pmol/L
- 41% through range
I believe you were on 62.5mcg Levo plus 5mcg T3 at that time and your endo suggested lowering Levo to 50mcg and increasing T3 to 10mcg.
Your TSH has risen to 1.5 so that's not good
Your FT4 now at 15 (12-22) is 30% through range so has decreased which would be expected with the change of doses.
Your FT3 at 5.24 (3.1-6.8) is 57% through range so not too bad.
If you, like me, need a higher FT4 then personally I would take it slow and increase Levo by 12.5mcg for now, leave T3 as it is, retest in 6-8 weeks and see how you feel and what your levels are like then. You can then decide what the next step should be.
How is your endo going to react to you tweaking doses yourself?

I think Endo had mentioned in last appointment if I felt ok on the 10mcg T3 to up it to 15mcg when I was ready but I think upping the T4 like you have suggested is more logical and also my TSH has risen which your right isn’t ideal, would make sense why I feel really sluggish again!

I've just done a thyroid monitoring with Medichecks and had a bit of a shock. I knew I wasn't feeling right, more tired than usual. My FT4 has gone from 75% down to 45% through range (19.5 down to 16.5) but FT3 has increased from 6.1 to 7.02 (3.1-6.8). Same doses, not Hashi's, so it's puzzling. However, I'm waiting for an iron panel result I did yesterday to see what's happening there, it might throw some light on it. So you can see for me that I need the higher FT4 but the higher FT3 came as a real shock. (I need to do some tweaking of doses to see if I can get a better balance.)
Maybe you do need a higher FT4, it will be interesting to see how it goes for you. It's such a pain, all this tweaking/testing
