Does anyone have any idea why my hemoglobin and ferritin have dropped, but my iron, saturation, and TIBC have improved? I haven’t had any treatment.
6/29/2018
Hemoglobin: 10.9 [L] (12-16)
Ferritin: 8 (8-252)
TIBC: 410 (250-450)
Iron: 122 (50-170)
Iron Saturation %: 30 (15-50)
11/14/2017
Hemoglobin: 12.1
Ferritin: 13
TIBC: 469 [H]
Iron: 81
Iron Saturation %: 17
I’m annoyed because the NP didn’t recommend anything, not even oral iron. Said she didn’t think my numbers would produce symptoms and that if my hemoglobin was in the 9s, then she’d be concerned.

This is common with hypothyroidism and especially Hashimoto's
For full Thyroid evaluation you need TSH, FT4, TT4, FT3 plus TPO and TG thyroid antibodies. Plus very important to test vitamin D, folate, ferritin and B12
Private tests are available. Thousands on here forced to do this as NHS often refuses to test FT3 or antibodies
Medichecks Thyroid plus ultra vitamin or Blue Horizon Thyroid plus eleven are the most popular choice. DIY finger prick test or option to pay extra for private blood draw. Both companies often have money off offers.
All thyroid tests should ideally be done as early as possible in morning and fasting. This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip, GP will be unaware)
If antibodies are high this is Hashimoto's, (also known by medics here in UK more commonly as autoimmune thyroid disease).
About 90% of all hypothyroidism in Uk is due to Hashimoto's. Low vitamins are especially common with Hashimoto's. Food intolerances are very common too, especially gluten. So it's important to get TPO and TG thyroid antibodies tested at least once .
You need TPO and TG antibodies tested for Hashimoto's disease- hypothyroidism
TSI and TSH receptor antibodies are tests for Graves' disease (hyperthyroid- over active)
Low vitamin D, low B12 and low ferritin can all be linked to Hashimoto's /hypothyroidism
Have you had folate tested?
Low stomach acid is often underlying issue and results in low vitamins

All thyroid tests should ideally be done as early as possible in morning and fasting. When on Levothyroxine, don't take in the 24 hours prior to test, delay and take straight after. This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip, GP will be unaware)

How low was vitamin D, what dose supplement are you taking? Have you had vitamin D retested since. Often people are given too small a dose (unless given high loading dose) and levels struggle to improve
