What does it mean when your tsh is 0.01 and t4 is 21.3.

Doesn't mean anything at all, without more information :

* have you been diagnosed hypo or hyper?

* are you taking something for either?

* how much?

* have you had your FT3 tested?

* and, most importantly, can you add the ranges, please?

Without all that information, they're just isolated numbers that can't be connected in any way.

I am hyper, taking ptu. They have only done my t4.

In that case, it looks like the ptu is working. But, you still haven't given the range. Tell your numpty doctors it is imperative to do an FT3. test.
