First blood test results since moving to NDT (Thiroyd) - opinions please

Hi all.

Well it's been 2 months since I started moving from Levo to NDT (Thiroyd).

I was taking 150mcg of Levo, so I gradually worked up to 2.25 grains of NDT which I have been taking now for a couple of weeks.

I've just had my first lot of bloods done (only sent the test sample off yesterday and got the results at 16:20 today, excellent service from Blue Horizons Medical.

I've attached an image of my results, I'd be grateful if you learned folks could analyse the results for me.

As far as I can see, lots of antibodies which I assume confirms that I have the autoimmune diesease Hashimoto's Thyroiditis - my GP has only ever referred to it as "an underactive thyroid". My Total T4 is on the very low side of normal and my FT4 is categorised as low. The blood was taken at 07:30, last meds before this was 16:00 the previous day.

Does this mean that my dose of 2.25 grains is too low?

I was on 125/120mcg Levo (3/4days a week) and switch the ThyroidS initially I was fine on 1 and 1/4 grains for about 2 years but had to up it to 2.25 but started to feel odd and find 2 grains is working perfectly. Your results indicate you are still undermedicated (others will give more expert and detailed opinions on the results I am sure) 2.25 grains is not a very high dose most people took between 2 and 3 when NDT was the only treatment. You could try the TPAU protocol which advises building up gradually until you start to feel hyper and back off to what feels right. I have not bothered with blood tests I just go on how I feel and I feel pretty good - a lot better than on Levo.

Thanks for the TPA guidance, I had a look and it all seems quite logical. Didn't know it was the T3 element that causes the palps, so that's something new I've learned.

So my T3 level is good, excellent.

But if FT4 is low, and pituitary is asking for more T4 by way of the "highish" (but still within range TSH) should I up my dose to 2.5 grains and see how I feel? That seems to be what the TPA suggests.

I know what you are saying about dosing as per patient symptoms rather than blood tests, and to be honest I would rather be slightly under than over medicated for sure.

Symptom wise, my main issues still are getting tired quickly, concentrating/brain fog and finding it extremely difficult to lose weight. I do feel like I have more energy on NDT though, which is great. I should add that as an overweight 14.5 stone, 5ft 11in, 46 year old male, the tiring may just be due to my low activity (desk job) lifestyle. I find weight loss very difficult especially as I have exercise induced asthma.
