It wasn't when I saw one, but that was a good ten years or so ago now. Contact the endo's secretary, or the private hospital/clinic that they work from to ask.
Depends, some do some don't, call the Endos secretary to find out. I saw one at a Spire hospital, I emailed her all my details and history beforehand and she was happy to see me without.
I went to Spire with a GP referral, BUT they were happy to see me without one (originally I thought GP might be awkward so I queried it with them). That was a couple of months ago.
Also had no issues with Nuffield in the past.

Not so rfu, it's not at all unusual to self refer.
