150mcg of levothyroxine is a very high starting dose. Was your wife hypothyroid before becoming pregnant?.

Has you doctor checked your wife's thyroid antibody level because it could be an autoimmune thyroid disease if her TSH swings high or low. If you have the ranges of the blood tests it would be helpful as labs differ throughout the country and it makes it easier for members to comment.

I assume your wife was NOT hypothyroid until the doctor checked her thyroid levels, i.e. TSH, T4. If so, it is usual for a dose of 50mcg to be started initially and increased about every 4 weeks to bring level down. Someone who knows more than me will respond.

Ask doctor to check her antibody level as they cause the TSH to rise and fall as they send out too much thyroid hormones and then too little. It is an important check.

If we take too much levothyroxine initially it can cause very unpleasant symptoms, ie. too hot, palpitations. etc. This is a previous post which I hope will be helpful. Has your wife been referred to a Specialist and/or midwife.

It may not be too useful to you but I will look for another link too.

Levothyroxine is taken first thing with 1 glass of water. Not eating for between half/one hour. Do not take levo on the morning of a blood test which you should have as early as possible as TSH is highest then.

If you have high antibodies the treatment is the same, i.e. levo.

You may want to read the following link alone so as not to worry your wife.

I believe that the TSH should be around 2 for pregnancy but someone will confirm.

Ammru, There's really nothing more you or your wife can do other than make sure she takes the Eltroxin regularly, one hour before food or two hours after food, two hours away from other medications and 4 hours away from iron, calcium and vitamin D.

Her levels were, and still are, high which can increase the risk of miscarriage but the high dose of Eltroxin your wife is taking means your baby will be getting the thyroid hormone it needs for good development.

Treatment is usually started at 50-75mcg but I think your doctor has prescribed the higher dose to ensure the baby's needs are met. Your wife should have a follow up blood test 4/6 weeks after starting Eltroxin in case a dose increase is needed.

I hope the pregnancy goes well.
