Foxyeyes, it will be the Hashimoto's attacking your thyroid which causes swelling and tenderness. When the attack ceases your thyroid swelling and soreness subsides. Gluten-free diet may help reduce antibodies and the frequency of attacks.

That's interesting. I had several episodes of this when I was on T3 only, accompanied by cold like symptoms - and the lump was exquisitely tender. No-one could tell me what it was, nothing showed up on ultrasound and, having been assured it was nothing to do with my thyroid, I was referred to ENT, diagnosed with acid reflux and recommended to try gaviscon!
Strangely enough, having been in situ for nearly three years, the lump disappeared overnight when I took my first 'real' dose of levothyroxine (75mcg) and it has not returned.
I do not have Hashi's.
Anstey, thyroid can swell because TSH is high and flogging the thyroid to produce more hormone, when someone is pumping out oodles of hormone because they are hyperthyroid, Hashi and Graves attacks and plain old infection caused by a virus or cold.
