Hi I have burning in the rectal area. I have IBS with constipation. I had 2 colonoscopies within 3 years from 2017-2019 and all good. I was having a uncomfortable pressure gas sensation on my left side above my colon for a long time. That’s why he did the colonoscopies . Then I first experienced burning in 2022 after running a 5K. Lasted 2.5 months. Went to Gastro and Urogynocologist for internal exams and no issues found. The urologist said it might be a perennial body or sphincter nerve issue, which is a pelvic floor issue. If the muscles are not relaxing in your pelvic floor, they can press up against nerves which can cause a burning sensation . I went to a physical therapist since I also had a nerve issue/pain in my buttocks area from the run which I thought is the reason why I had the burning in my rectal area and after one session of PT and doing some exercises at home that nerve pain went away and then a few days after that, the burning sensation in my rectal area went away as well. In January 2023 I decided to do a two hour walk with 10 pound hand weights and a few days after that, I experienced really bad burning sensation again. I went back to the Urogynecologist and another internal exam revealed nothing that he can feel.
I have a appointment scheduled for the Gastro doctor at the end of the month and I have a physical therapist appointment scheduled for next week. I have no clue if the physical therapy may help again for a possible pelvic floor/burning issue. I have very small hemorrhoids. I had very, very large hemorrhoids removed many years ago. It was a very painful surgery. I only feel relief when I lay down. When I wake up in the morning, the burning is not as bad but as the day progresses, it gets worse. It makes me feel like I have to have a bowel movement or urinate and I really don’t so it may definitely be nerve related. I know how frustrating it can be when you don’t feel well, and no one can give you an answer.

Hi , I think it's a bit hard to pin point the cause of this burning sensation your describing.The only thing I can think of is you maybe allergic to somekind of wash powder/liquid.
I know it sounds like a daft Idea but I have actually come across it where clothing can irritate certain areas of the body.
With it being in a hot area as the rectum it is possible that chemicals can effect certain skin.
Also without being too personal when you have had a bath,Do you wipe yourself or do you dab the areas? My GP has said that if you dab in areas such as this then it doesn't irritate as much and burn.Which is also another thing.Does your shower gel or soap agree with your skin? You see I can only wash with certain Lynx S/Gels because it dries out my skin. I'm just trying to find a solution to your burning sensation problem.Maybe something to think about , Certain chemicals can be the cause.
I feel it’s a nerve issue. I’m fine when I sleep and wake up. I only use natural chemical free and organic personal washes. I shower daily. This happened once after a 5k and once after running and walking with heavy hand weights.
I have exactly the same symptoms, and it is painful and bewildering and have suffered this for months the pain is excruciating and almost cripples me when I walk, also my perineum and underneath my bottom feels so sore and hurts to touch that area, at last I have an appointment to see a urogynecoligist tomorrow morning which is the 4th of March, I will keep you informed on what he says is causing the problem and like yourself I also have pelvic pain, ps t have also thought is it a nerve issue.
