latest results & pericaridal effusion

I’m 50 yr old female with issues, RLS, Hashimotos, Autoimmune Atrophic Chronic Gastritis, intestinal Metaplasia, Iron Deficiency Anemia w/transfusions, B12 deficiency, and now this horrible hip and joint pain. I have now seen an endocrinologist and had another set of tests. My TSH is not measurable at < 0.02, my FT4 is within range at 20.9 (11-22.7) and my FT3 is 5.9 (3.5 -6.5), much higher than it has ever been before. My previous results on 15/04 were TSH 0.02, FT4 23.6 (12-22), FT3 4.7 (3.1-6.8). These

Thyroid test results
I’ve been taking 200mg of thyroxine for most of my adult life. Now the doctors have reduced to 150mg I returned to the doctors to say I felt awful. All my symptoms were returning. So they issued another blood test. They have came back saying my TSH level is too low and reduced my dose even more.

I have my appointment with the endo this afternoon. I have read that if you’re on T3 and T4, your TSH should be within the normal range. my T3 was 5.7 (with 6.8 being the top of the range) and my T4 was 21.3 (with 22 being the top of the range). My TSH is suppressed. Please can someone tell me if

liver Mets, Pluvicto failed
diagnosed 2021, spread to distant lymph nodes. Castrate resistant within 6 months. Had platinum based chemo. Stabilized for bit. 4 Pluvicto treatments. PSA went from 4 to 70. Latest scans show new mets to liver. Genetic testing shows ATM mutation. Where to go from here?
