diagnosed 2021, spread to distant lymph nodes. Castrate resistant within 6 months. Had platinum based chemo. Stabilized for bit. 4 Pluvicto treatments. PSA went from 4 to 70. Latest scans show new mets to liver. Genetic testing shows ATM mutation. Where to go from here?

I’m so sorry. I know here are trials going on for Actenium which may help you but honestly I don’t know anything yet about liver mets. I will be praying for you and a successful treatment
My husband was diagnosed in 2022 and had his first Pluvicto on the day after Xmas. Now it has spread to his meninges (area between skull and brain). He’ll get scanned after second Pluvicto 2/6. I’m worried it’s not working.
Did you have any relief from pain after your Pluvicto treatments?
I can answer to that Izzy and thank you for hanging in there with your husband. I've seen your posts. Difficult time. I don't want to hijack the posters post so I'll leave it at that.
It used to be pain in my scapula, shoulder and arm but for a year or more pain in my femur and hip is the "canary in the coal mine". Pain increase with progression and abates when treatment is working.
Quite some time after my second Pluvicto infusion the pain in femur and hip abated. I still have a "hitch", a feeling I better be careful with movement but the pain is pretty much gone.

You could request opinion about starting olaparib, rucaparib,or talazoparib, alone or in combination with ADT plus zytiga or xtandi. These combination work the best for BRCA mutations but some ATM mutated cancers may respond.
