13 year Battle with Stage 4 PC
He had
Provenge
which helped for a few months. Then he was offered a clinical trial of zytiga + Predisone with Xgeva every 6 weeks. He was on this trial for a year until lymph node involvement made him ineligible.

Provenge experience
So I also got
Provenge
immunotherapy in May. It was a bit of a hassle due to logistics but all in all a big nothing burger. First I had to stop my existing treatment, Xtandi.

Has anyone used Dr. Gary Onik (Dr. Hope)? Want opinions.
Provenge
, Xgeva, Lupron (now castrated for real since my dog said it was no big deal...and he was right). The last 8 years have been some party!

Any thoughts on why PSA rising but mets status stable?
I am writing for my dad who was diagnosed with PCa in 2010, after using ADT, Abiraterone, Olaprib,
Provenge
(Car-T) and now his PSA is about 5.2, with 5 mets on lliac and neck lymphatic.

When is Provenge indicated?
I've only recently read about
Provenge
and I would like to know when insurance (Medicare) would pay for it. Officially my husband is castrate sensitive (but PSA is trending upward on Orgovyx).

He has undergone surgery, radiation, hormone therapy (Lupron),
Provenge
, Clinical Trial. Chemotherapy, Lutetium 177 Treatment in Germany (4) cycles and is now on Xtandi. His oncologist does not hold hope that the Xtandi will bring down is PSA.

Keytruda update
Completed chemo,
provenge
, Zytiga, Xtandi and an ARV 110 trial. Currently on Lupron and keytruda. A second Foundation One DNA test showed high mutational burden with mmr 2 and 6, cdk 12 and pten loss. As a result his MO started him on keytruda.

Just read a private chat from 2 years ago...This was my reply. Lu-177 thoughts (then/now) and the reality of "life in Omaha"
****July 2/19****I did do
Provenge
(fairly new) over the past summer. If I don't get a "miracle" remission on just Lupron and Xtandi (and BIRM and melatonin and mushrooms and lighting incense and chanting)....then I am probably seeing the Lu-PSMA kind of thing in my near future.

***Six months scans results are back ***
My treatment so far is Zytiga, Eligard, Xgeva which has since been stopped because it caused dead bone in the jaw that had to be removed,
Provenge
infusions two years ago.

My doctor wants me to try
Provenge
for my gradual PSA increase. Right now at 6.4 . But in a upward trend . Has anybody had success with
Provenge
treatments. And will Medicare pay for it ? Thank you all for your feedback.

Thinking About “Median Survival” in Clinical Trials.
The median survival for the
Provenge
arm would be at 38 months, the date half those on that arm were dead.

Off label use of Mekinist (Trametinib)
Over next 3 years
Provenge
, Zytiga, Xtandi and Taxotere. By summer 2014 extensive in bone marrow. Xofigo started in Feb 2014. Became anemic and could only be kept alive with monthly blood transfusions. Xofigo stopped in August. PSA over 120. Bills chance of living another 6 months bleak.

I've completed
Provenge
and Xtandi so I believe Chemo is next. I'm still open to suggestions if I need to move on to a new treatment. I appreciate all of you and the collection of information I find on this site.

Continue Lupron after continuing PSA rise?
He has had
Provenge
, Xtandi, Dosetaxel, Cabazitaxel and all worked for a good time, but his PSA has been steadily and firmly rising and new bone Mets for the past few months.

What to do when in remission?
Provenge
, Bicalutemide, and supplements. Supplements are hard to choose since there are so many and costly. Is there such a thing as top of the list for supplements. For T to stay low is a disappointment and a blessing. I think. Urologist and ONC suggests going off Dutasteride and Finasteride.

DEXA Scan - how often?
His treatments: Lupron, Taxotere (6 rounds when first diagnosed), Zytiga and prednisone,
Provenge
. PSA has remained at <.1. No pain, he's still working full time. He has the usual side effects from the treatments, fatigue, weight gain, but nothing serious. He gets a CT scan once a year.
