Has Anyone taken Xtandi as a last resort treatment for MCRPC and what were results?

My husband has run the gamut on Prostate Cancer Treatments- Surgery, Radiation, Lupron, Clinical Trial (Panther), Provenge, chemo, and Lutetium 177 treatment in Germany for 4 cycles. The Lutetium worked with bringing PSA down from 1350 to single digits for about 9 months - now it is on the rise again. His MO put him on Xtandi and the side effects are very difficult. We do not know yet if it is working. Just wanted to check on other experiences. We have been battling this monster PC since 2008. We are feeling down. He has heavily involved bone Mets.

Hi Poppymom,

Very sorry to hear your situation. This has, is taking an emotional toll on you I bet. It is so great of you to do what you can for your husband. Your wording in your post speaks possibly of your emotions. I hope and think that no ….Xtandi probably isn’t your last option.

Tall_Allens suggestion of discussing Xofigo (Ra-223) is a good one and as he mentioned your husband’s bone marrow (blood test markers) will have to be good to undergo the treatment.

I’m no expert but chemo can be used more than once in some cases. (you mentioned he has had chemo once) It kills cancer and sometimes “resensitizes” so that treatment not working now might work after the chemo.

The chemo combination of Jevtana (Cabazitaxel) and Carboplatin might also be worth discussing with his oncologist. If your husband’s health/fitness isn’t great you can still start chemo and quit if it is too much in side effects.

For now, maybe discuss with Oncologist: see if the Xtandi dose can be reduced and keep a close eye on his PSA and pain level to determine if it is working?

Zytiga (Abiraterone) as some have mentioned may be worth discussing as well.

I see now you have posted looking for good Oncologist in N.C. I can’t help with that but I am sure you will hear from others here. Sometimes Oncologist’s at college Universities are very up to date on trials and soon to be finished trials and aware of new treatments.

I went on xtandi in a clinical trial. It worked well. My PSA was low for 4 years. I did not experience any severe side effects but then everyone responds differently to drugs. There are more ADT drug than just xtandi. If the side effects are causing pain and other symptoms ask his doctor is switch to another drug, i.e. zytiga.

You might also asked about Provenge if the mets are in soft tissue.

Is he taking a steroid with the Xtandi? Lots of options still and in the fight since 2008!!! Wow that’s impressive. Has your husband’s cancer been tested for genetic markers that may help guid treatment options?

My body responded much better to Zytiga vs Xtandi, and both my Testosterone and PSA also reacted better. Each human body is different. We all present, and react differently. Also Lupron w Prednisone is different than Lupron with dexamethasone. Best of luck,

Spyder can i ask did u switch to ZYTIGA because xtandi stopped working or because of side effects 🤔 Many thanks

Have you and him did diet management of prostate mets? Some foods result in increased mets and others decrease this. Primarily avoid sugar, beer, red meats, smoked meats, egg yolks there are others. Find a list that decreases mets. Anti-infam diets are good
