Should I cancel salvage radiation and ADT
I had a radical
prostatectomy
in 2014, Monitoring of my psa showed increasing levels; from 0.02 to 0.57 in Feb 2021. So it took 7 years to get from 0.02 to 0.57 I commenced ADT treatment 3 months ago, July2021. My PSA is currently 0.02, back to where I was 7 years ago.

ART PCRI Postponing Hormone Resistance and Perineural Invasion PNI
A: Perineural invasion (PNI) is another in my opinion is a somewhat antiquated marker on reports on needle biopsy, pathology reports and on radical
prostatectomy
reports. 1.PNI addresses the fact that, microscopically the Pathologist could see the cancer cells inside the prostate growing along the surface

Lynparza (olaparib) +Zytiga (abiraterone) better than Zytiga alone for mCRPC regardless of BRCA status
It DOES apply to men who have progressed to metastatic castration-resistant PCa after previous
prostatectomy
and primary or salvage radiation, and are chemo and second-line hormonal naive. The surprise is that Lynparza works so well in combination with abiraterone.

2 years of ADT counting from end of IMRT versus first injection 6 months before?
And to further clarify since I haven’t posted in awhile, I had 1 positive lymph node up on the bladder and 6 weeks post
prostatectomy
my PSA was 0.41 prompting the IMRT.

My Oligometastatic Journey Continues…
In the summer of 2018, I had a
prostatectomy
at MD Anderson. They were hoping for a durable remission or a cure. (Had a pacemaker installed in 2019 - had some electrical issues possibly due to the radiation…) In January of 2020, we decided to give an ADT “holiday” a try.

I understand that PSMA technology https://www.fda.gov/news-events/press-announcements/fda-approves-first-psma-targeted-pet-imaging-drug-men-prostate-cancer is not useful for patients with post

PSAs below 0.2. Mine is 0.02.

It is clearly the future of robotic
prostatectomy
. Just make sure you are in the hands of an experienced surgeon.

When to start

Retzius sparing prostatectomy, 1 week later
Hi all, I still need to update my journey blog, but thought I would give an update as it has been a week today since my procedure at Georgetown. The procedure itself went really well. Turns out my prostate was 100 grams. Pathology came back showing 4+3, versus original 3+4 from biopsy. No positive
