Hello, I’m new to this community as my husband was recently diagnosed with MS.
We love our MS specialist but she cannot prescribe
opioids
for his breakthrough pain in shoulder. Went to a pain doctor who is trying to get him to swap from Baclofen to Tizanidine, which it took him months to adjust to.

She is also good about
opioids
if needed PRN. BTW I am 70 years and my father developed neuropathy in his 80's but absolutely hated the sedative effects of Gabapentin. He and I have similar responses to meds in general, so I am somewhat apprehensive about gabapentin.

He thinks I’m mad not to continue with Pramipexole and understandably he is not keen to give me
opioids
. But, he is starting to listen to me, so there is a chance. My question is this: what can I ask him for to guide me through the last weeks of tapering?

The RLS nightmare
In 2013 i was put on disability because I have flat back syndrome and it has caused chronic pain for over 20 years non stop 24/7 and I am addicted to
opioids
. On June 20th of this yr I received a dbl laminectomy on my l4 and l5 vertebra and the knife was removed.

More cold Turkey!
I don't feel like I can take much more.My doctor, who is keen to help but wary of
opioids
prescribed me an antidepressant called nortriptyline 25mg. I took one and it nearly killed me. I had the most terrible 24 hrs and I will not be taking that again. Is Nortriptyline any good? am I being a wuss?

Interesting update for those in the UK whose GP practice has Buprenorphine red listed
Next I had to find a consultant who would be open to prescribing
opioids
. I decided to take a risk on Dr Robin Fackrell at the Sulis Hospital in Bath. He charges £265 for an initial consultation, significantly less than Professor Matthew Walker (£360). I found Dr Fackrell to be very receptive.

Looking for some guidance please
I know about the groups guidance to wean off slowly but am concerned about getting through this as things are already so bad and when I tried last fall I was going bananas. 2) I never hear of anyone in the group using hydromorphone yet I hear of people using many other types of
opioids
such as Buprenorphine

Oxycodone - the way forward or a dead end?
I approached the subject of
opioids
. It was clear that as
opioids
are not on the formulary of the NHS hospital where she works, she could not prescribe them - nor would she want to - But she agreed to ask my GP to prescribe Oxycodone-naloxone ( Targinact).

I get the fact of being on long term

can affect lots of things . But !!!! Why now I'm 60 years of age and I have changed in a year ,my personality was always positive have a laugh at my expense, enjoy my grandkids live life as I have already been through so much in my 60years .

Obviously I'm not thrilled to be now offered the opportunity to take Pramipexole however I am aware that the Mayo advice is, if there are problems with Gabapentin/Pregabalin, to try DAs as a second line ie before
opioids
.

Dipyridamole didn't work for me
The standard advice for weaning off DAs only mentions using
opioids
as a last resort. I don't know how I could survive one night, let alone a few months, of no DA (or reduced dose)and no opioid. I'm generally fairly pain-tolerant, but maybe I'm a real baby when it comes to RLS.

Pelvic floor therapy after menopause
I am taking another benzodiazepine for sleep/anxiety (clonazepam) and I'm not sure if the doctor prescribing that will want me to mix the two, especially since I am taking
opioids
and baclofen, which both tend to slow respiration.

Low Dose Naltrexone (LDN)
Multiple medicines later and serious consequences from that I was told I was hyper-sensitive and stayed away from
opioids
. I take Co-codamol, Nortriptyline and Sertraline and some Tumeric. Over the weekend I had increasing abdominal pain and ended up in A&E for eight and a half hours alone.

Any last advice before I embark on the battle to try Buprenorphine? Should I increase my pregabalin dose?
In the meantime I've started to wonder whether I've done everything possible to avoid the need for
opioids
. My main question is about my pregabalin dose, and whether a higher dose might be worth a shot.

Does anyone have direct experience of taking Targinact or similar
opioids
, and can reassure me that it's ok - or as ok as it's possible to be given it's an addictive painkiller? Thank you all so much!

I have something for nighttime but the US is so paranoid from the abuse of
opioids
that only a miracle would help me find and get an appointment in the next 6 months with a doctor that would prescribe them.
