Largest real world study of acalabrutinib and ibrutinib over 3 years, shows acalabrutinib has improved discontinuation rates
We've had several posts in the last 24 hours about the U.S. government's plans to negotiate a price for
ibrutinib
. I mentioned https://healthunlocked.com/cllsupport/posts/private/149916982/

Efficacy and safety of new‑generation BTKis (primarily acalabrutinib and zanubrutinib) in CLL/SLL: a systematic review and meta‑analysis
Based on the results of these studies, zanubrutinib and acalabrutinib are preferred over
ibrutinib
due to their favorable safety profile, and zanubrutinib has superior efficacy compared with

Ibrutinib failure
I know there would be a shelf life for the
Ibrutinib
which I’ve taken since 2019 and the majority of that time on the lowest dosage which seemed to be keeping my bloods in acceptable range. Still it’s been quite a shock, was I getting too complacent? Any advice would be appreciated. Thanks Nick

What to do? a very enlarged spleen & very little money for treatment
My options may be 1)
Ibrutinib
, Acalbrutinib or Zanubrutinib, or Obinutuzumab plus Venetoclax (following NCCN guidelines) or 2) removing my huge spleen (8 x 3.5 x 5.5") , a surgery not done as much now as in the past. The meds reduce the spleen, but are super-expensive.

Retuximab
When I was first diagnosed I had chemotherpy for 4 months, then I went on
Ibrutinib
which gave me heart palpitations, but seemed to do the trick as Ihave been in remission up until this year. I now have swollen glands all over especially my tonsils which I am having removed.

Switch ibrutinib to zanubrutinib
Is there any problem switching from
ibrutinib
to zanubrutinib? Why wouldn't that be a good idea?

changes in markers and new treatment
I achieved UMRD at the end of the trial of
Ibrutinib
and Venetoclax. I remained in total remission until my visit after my husband died from Alzheimer’s in Oct 2021. Since then it’s been downhill.

Update on Ven & Obin protocol
Overall I have found this protocol side effect free compared to
ibrutinib
where I had a lot of joint pain, muscle cramping, skin rashes etc especially the first couple years. Most importantly my numbers are all good - I had one blip with my platelets but they have popped back up.

How you can help make ibrutinib plus venetoclax fixed duration therapy a reality for Australians with previously untreated CLL
In March 2024, PBAC will again consider a submission to recommend a 15-month combination therapy of
ibrutinib
plus venetoclax for those with untreated CLL.

Warts and all
Starting on one of the first phase II trials of
ibrutinib
in 2012, now on acalabrutinib, it's been a good run. My big problem is the immunocompromise that accompanies CLL and its treatments, especially the FC regimen that damages bone marrow stem cells.

Petechia
I have been treated with
Ibrutinib
for 16 months and have responded to treatment really well. My white blood count was 148.7 when I started treatment now I am 14.7. I have Petechia on arms and legs. My platelets are 278. Just wondering what could be causing this.
