Not so sure I should be on Erleada
He also orders
Erleada
as he thinks I may be castrate resistant but not certain. 10/28 lab results - PSA remains undetectable. Testosterone is back <7. Free Testosterone noted as "low".
Erleada
arrives and I took my first two doses 11/10 and 11/11. OK so here's where my question comes in.

Zytiga Failed Me Bigtime
After going on Lupron and
Erleada
my PSA dropped to .20 in 6 months. After that it did nothing but rise back up to 120 about 4 weeks ago. After the
Erleada
started to fail I did 3 months of Xofigo, followed by 3-1/2 of Zytiga.

I know I'm lucky, but.....
Don't get me wrong, I'm not looking for something to do just so that I'm doing something, but i keep thinking maybe I can cut my dosage of
Erleada
....maybe that would stretch the amount of time it would be effective. Maybe repeat bone and DEXA scans?

PSA rising (but relatively low). Is PSMA scan advised at this point? Or is it too early to be helpful?
Fast forward: has been tolerating Lupron &
Erleada
very well, all things considered. Weight gain most concerning as he is already on 4 different medications to manage his blood pressure. January visit found PSA rising and March it rose again.

Hey lucky me - I now have 2 cancers
Open thoughts at this point
- the
Erleada
may be free to me as I have applied for a grant. I'm not sure I need it as it appears Lupron is doing it's job, but another tool in the tool box. Taking two different ADT is concerning to me, hence the consult on the orchiectomy.

Darolutamide & time to treatment discontinuation in nmCRPC

More Evidence Linking ADT for PCa to Adverse Neurocognitive Effects -Meta-analysis shows increased risk of dementia, Parkinson's, depression
The risk was even higher in men treated with newer androgen receptor signaling inhibitors (ARSIs), but the association was not consistent across the ARSI class: increased risk with enzalutamide (Xtandi) and apalutamide (
Erleada
) but decreased risk with abiraterone (Zytiga).

PSA rising - When to get a PSMA PET scan?
A year of Degarelix (Firmagon) + Apalutamide (
Erleada
) bought me more than 2 years of undetectable PSA. Now the PSA is climbing past 0.5 with a doubling time of about 5.5 months so a GA68 PSMA PET scan is probably the next move. I wonder how long to wait before getting the scan.

Apalutamide (Erleada) Only Lasted 6-7 Months! What Next?
After being on Apalutamide, Lupron, & Xgeva, since April of 2022, & PSA drop from 7.3 (extensive bone mets.) & a drop to 0.1 (for 2 mons.), PSA rose to 0.6 over 3 mon. w/o lab work because doing so well. So, don't know exactly when it started to rise but the rate of increase is geometric! MO wanted

Short-term androgen annihilation in non-metastatic recurrent men delays progression
If one has rapid PSA after salvage radiation, and no metastases on conventional imaging yet, there may be an advantage to adding short-term (8 months-1 year) therapy with a 2nd line hormonal agent (Zytiga, Xtandi or
Erleada
): https://www.prostatecancer.news/2022/09/short-term-androgen-annihilation-in-non.html
