Pain near groin, but no PSA detected?

MyGuy is having severe pain low, just left of his groin. It’s not always there, but today it was bad for quite a while. He had a bone scan of his pelvis and an mri last June and nothing was noted. His three PSA tests since June (with the last in January) have been undetectable. When pain was mentioned to his doc in January, his doc didn’t seem to have an answer. MyGuy has been on morphine for groin pain for about 18 months, but says this is different. His next appointment with his doc is in April.

I told him I’d check in with you all… Any ideas/suggestions? ( I believe I shared his initial diagnosis in his profile. Currently on ADT (Erleada) protocol. )

Has he had an MRI?

He did last June. There was nothing much there, but as Grandpa4 pointed out arthritis, maybe that is it. MyGuy said just now he thinks that might have been mentioned in the report.

Very reasonable to have some myofascial treatment to his piriformis bilaterally. Safe and the potential effectiveness is worth it

I assume he was checked for a hernia. Any edema in his left leg - evidence of a lymphocele? Is it tender to the touch? Does it diminish with Neurontin? Is he diabetic? Has he ever had a heart catheter inserted there?

I don’t think he was checked for a hernia, and it’s no to the other questions. We’re going to look into this. Thank you.

groin pain is often hip joint arthritis. Easy to get an x-ray

Maybe that’s it , and we’ll look into it. He’s fairly sedentary but does walk a few miles every day, with some 9 hole golf here and there.

my guess is only a guess. Inguinal hernia will cause deep pain in the groin. The intestine is trying to push thru that canal. It can be pushed back in place often by lying on your stomach. I used to do it when lying on a surfboard. The pain goes away at that point. He can self diagnose by pushing down there and coughing. Surgery is quick and usually successful. Recovery is about one week. One month for athletics. Mike

I just read about it, and it does seem to describe his symptoms. He struggles with serious constipation due to the morphine, it seems this would certainly aggravate it. Thank you!

Constipation and bowel issues can cause pain too. So can swollen veins. Is he overweight or retaining water? Has he considered a motility drug?

May just be the inevitable decline of age.

Had similar, mentioned it in passing to my Radiologist. He reviewed scans and concluded it was a hernia waiting to happen. Didn't sound serious and only softly aches every few months when resting in bed. Didn't pursue.

Me too. I have one that has been marginal for 8 years. It comes and goes as far as discomfort goes. The modulation in discomfort is sometimes just stuff moving thru the intestines pressing against the weaknesses. Not fully broken but weak

For what it's worth my husband had the same kind of pain in that area and was diagnosed with a hernia at the same time he was also diagnosed with PC.

I've had a couple of hernias on my right and left sides. In fact I have one now. Just push down in that area and if it hurts it probably is a hernia. If you see a lump there (don't get excited) it's just the hernia popping out (especially if he's lying down).

BTW hernias always come back.... They put door screens (closed) to prevent them from coming back, but I think they rust away....
