Is ADT routinely given after prostatectomy

Dental Issues with Finesteride
I am 7 years post full gland HIFU surgery. Quality of life remains good. I've been off ADT all this time. PSA has been slowly rising and hit 2.8. My urologist put me on finesteride. It prevents the conversion of T to DHT. Five weeks after starting on it I experienced an onslaught of dental problems.

Second Recurrence
I was originally diagnosed in December 2018 with stage 3 high-grade primary peritoneal carcinoma. It recurred in December 2022. I had the same chemo both times: carbo/taxol. It appears that I have another recurrence because my CA 125 was 8 this past November. Then, it rose to 22 in December and today

Docetaxel and Carboplatin
I’ve been on the PC journey for 12 years. I’ve had lupron and all the standard “poisons” that we get, including Pluvicto. My PSA is now up to 99.5. I have bone mets in my lower back and sacrum w periodic pain that’s controlled w pain meds. My oncologist is recommending a chemo concoction of Docetaxel

Should Perineural Invasion (PNI) change my Radiation Treatment Plan
Hi everyone. A quick background. Age 59. Was diagnosed in Nov 2023 with Gleason 3+4=7 Intermediate Unfavorable (10 of 15 cores positive) and tumor in both side of prostate. PSA 5.9 Decipher .49 Bone Scan showed no metastasis Met with Multi Discipline team at Inova Schar Cancer Institute in Fairfax

Right phrenic nerve paralysis
Path from RP showed extensive perineural involvement. No other risk factors for phrenic nerve paralysis. Haven't talked to MO yet, but sure concerned it's tumor invasion. PSA still <0.04.

At what point for scan ?
Hi. Currently just had # 4 chemo. Live in Canada so PSMA has never been done . Bone scan and CT scan. I'm wondering at what point it would be good to get a scan ( they already said it would probably be a CT scan as that's all we have in my province and eventually another bone scan which He has to fly

My MO suspects that I am non-metastatic, castration resistant but will not approve any treatment until metastases develop.
Gleason 9 at diagnosis, BRACA 2 germline and somatic. RP five years ago but cancer escaped the capsule; PSA of around 2.5 after surgery. Had radiation to pelvic area a few months later along with Eligard injection. Renewed Eligard after 3 months and PSA went down to undetectable. T was close to zero

Starting Chemo again, Cabazitaxel Has anybody experienced any side effects that have been unusually bad?
