have had brachy HD and external radiatio then started ADT Decided to have testicles removed instead of taking ADT will probably continue with Anrogen receptor blocker like bicalutamide Question is weather side effects have lessened without the ADT drug I know that many of the side effects are from the loss of testosterone in either situation but there should at least be some improvement from not taking an ADT drug ? Anybody done this ??

You will have a permanent orchiectomy instead of temporary (1 year) of ADT?

Your right about that but at75 it isn t a big issue except I guess for bone loss etc I wish I could see in the future to know if I can go off adt in a year I am in remission as I had Bracy HD and External beam on prostate with 4 subsequent bone mets that we immediately radiated with SBRT My body responds well to treatments and I may never have future mets etc I could go off Orgovyx and just take bicalutamide and see if psa rises before making that choice?? I just need to reduce the chemicals I take as My body doesn t tolerate them well usually headaches Thanks

Oh, I didn't know about the bone metastases. It is permanent then.

Thanks I need to decide to remove testicles and just take Bicalutimide instead of taking ADT as well the rest of my life

I've read the earlier discussions of this issue on the group, but have never asked: which doctor performs the orchiectomy? A urologist?

At least in my case, “yes”… urologist 😊

I can’t tell if my side effects lessened or not. That they didn’t increase is easier. Long term effects such as cardiac involvement are what I wanted to avoid. So far two years litter and I can say no increased ses. Hardly any hot flashes which is great

Thanks That is very helpful. I have read somewhere that the effects on a man s body are better after testicle removal than long term ADT

The side effects have dropped for me dramatically, specifically mood swings and hot flashes are all but gone. I take Nubeqa.

The absence of Testosterone causes most of the side effects and your adrenals will still produce about 5% of you overall testosterone production, so why I'm feeling so much more "normal", I can't explain, but I do and it's not by a small margin.

Thankyou that is very helpful My plan is also to take Nubqa or bicalutamide

Best of luck and please take care in looking at all of the side effects for both drugs including contraindications with other medicines you might be taking. I use Medscape.com for this and so does my doctor.

I was being driven crazy by Lupron and after the Orchiectomy 3 years ago things have been sooooo much better. You still need casodex/bicalutamide or similar until they no longer work for you (that's where I am now). Those drugs have side effects so you will not be SE clear. Just QOL much better, in my experience.

glad to hear that thanks

I decided to have an orchiectomy because I have Mets and would have to get Lupron injections for life, I just didn't like getting the injections. I didn't have many side effects before and don't have any now.

I decided for complete orchiectomy and today I can say, it was a good decision. Less side effects and no injection every three months.

Thanks I expect to do the same as the fewer medicines the better
