I have found that dietary changes often help with acid reflux in my case.

ie eating more of the alkaline foods and less of the acid foods.

It might not work for you, but if the next prospect is surgery it may be worth giving it a try first. Others have said that eating avocados helps to neutralise acid. I have also noticed a positive effect with them.

I also take Gaviscon when it gets bad

My advice would be to avoid surgery if at all possible. Have you had a dilatation of your oesophagus at any time. That may help. Nexium (esomeprazole) is the next stage from lansoprazole. It does help and I was put on this medication when Barretts was diagnosed. I also have Crest and have to have a dilatation from time to time which does help.
