I’ve done a search on our blog but didn’t find anything specific…It’s been about 8 months on Piqray, and after an array of all the early side effects, I’m now dealing with painful finger tips and nails. I can deal with the thinning hair and fatigue but it makes me so sad to give up a lot of my hand dexterity. I think it’s the hand and foo5 syndrome that is associated with Piqray…but no blistering or redness 🙏🙏🙏🙏 but my nails are peeling back to the nail bed and I have painful fingertips. I’m taking 900 mg daily of gabapentin but I’m wondering if it’s working 🙄. I
Is anyone dealing with similar issues and have yiu found any relief beyond gloves and hand creams ? Best wishes to you all…and fingers crossed on the introduction of a new drug. Itovebi (inavolisib) in combination with palbociclib and fulvestrant as a new treatment for advanced breast cancer:
We all need some good news!.

Sorry to hear you have that side effect. Is there any chance of lowering the dose? If it is hfs I had that while on Xeloda. Mostly my feet but one particular cycle it was all about my finger tips being sore like you describe. I couldn’t open plastic food containers with the thin edges! It was crazy really. Lowering the dose helped immensely, so perhaps ask about that. We do need to find that QOL balance!
I think HFS is only associated with Xeloda/Capecitabine. HFS is where the plantar of the hands a feet burns a swell and peel I think. Not something I want to sign up for after having to deal with this syndrome which should be called Nail and Fingertip syndrome., But my experience with Piqray is the same as yours Drangonfly. My fingernails are also pealing back to the nail bed. It’s awful. Sore. Tender. Broken. Peeling. Yucky Yucky feeling. I’m hating this drug.
thank you timtam! On top of my other woes I got covid just before thanksgiving and am lying low. Thankfully, it’s a mild case now and am feeling much better. I think the lower Piqray dosage (250) is finally making a difference. I use a topical nail hardener and the several coats give the nails protection and strength. Most fingers are recovering though the right thumb and forefinger are still very short. Ugh…it sure limits dexterity and hand strength…best wishes timtam…!
Oh wow! you are describing the exact problem that i have been experiencing on Piqray. I am now on cycle 19 (200mg)and my poor fingers! I think because the nails are so far back, the finger bed is exposed and is so painful. It splits and bleeds and flakes off and the nails themselves are so paper thin.
I have been trying an anti-fungal laquer as i thought it may be due to some secondary fungal infection. This seems to be helping, but it is early days. it does make the appearance better though. I am ashamed of how awful they look sometimes.
For the delicate finger tips, I bought some silicone finger protectors which i use at night. i cover the nail and finger with eczema cream and wound healing gel and let it sink in over night. I alternate between fingers depending on which ones are worse. it is helping slowly.
Gosh, Laskia, you are certainly struggling with this. The Nailtique is really helping me...I don[t think it's something special about this particular brand..but rather the fact that I have a thick coating on the nails and they are protected from everyday actions. Your nightly ritual is interesting...I just use Eucerin lotion as a nightly cream on my torso where I still get the occasional rash eruptions. this has era in it so it's especially helpful to Piqray issues. best wishes and thank you for your response!
