Hello Sir/Madam,

Please find the patient details I have submitted to participate in research and treatment of progressive supranuclear palsy.

1 PSP – Progressive supranuclear palsy 4 Year

2 Falling back side, imbalance, anxiety, body tends to move back while walking, dementia 3.5 Year

Choking, gagging or gasping for air. causing cough 1 Year

3 Legs spreads if walk too much (500 meter)

4 slurred speeches, less and blurred visibility

5 UTI 8 months

6 Diabetes 15 years

7 High Blood pressure 6 Year.

8 Opening mouth very less, not able to sit properly, bending back side while sitting.

9 Dystonia on the right side of the body, Legs will lift up and bend when the patient tries to stand.

10 Right side Eye has swallowing and water.

11 Stiffness, sometimes legs are not moving when a patient tries to move and turn .

Patient Name - Phoolchand Ram Pal.

Age - 64 Year.
