Can anyone inform me approximately how long before ones CHC is due to end,the review takes place. have been on to the CHC people but they are non committal. Also if someone could inform me on the format for example what kind of questions do they ask and how does it differ from the original application and any tips you might have,I would be much obliged. There is a review after six months which is most often a check to see if the care plan is working and that the service users health is much the same. Often they don't use a format, but they might just use the DST.
There is then a review at one your and every year there after. Those reviews generally use the DST format with all of the evidence gathering from involved services.
Mostly CCGs are so overwhelmed with work they seem to be leaving those reviews that are fairly certainly not absolutely necessary. I suspect they make a phone call to the key agency involved and ask them if there are any significant changes.
This will explain their reluctance to say.
I just leave them be, don't bate the sleeping dragon as it were.
They will give notice of the review if and when.

Hi edgie, I as told by the assessment team that they would only remove funding if there was significant improvement in the patient. Unfortunately with this illness such miracles are unlikely. My wife's full (2 to 3 hours) assessment was changed to a fast track assessment after about 10 minutes and I was told she would be reassessed after 3 months. That was over 3 months ago. I agree with Kevin, do not "poke the bees nest" (another cliche). Rob
