Can anyone tell me the benefit for having as DS1500 form? My mum was really ill recently and the medical staff said it was time for this form to be completed. We live in Scotland so unsure if it only pertains to us. Thanks

Sorry to be the barer of bad news. You get the ds1500 form completed when the person is near the end of life normally when they think death is approximately 6 months. If sent off with an attendance allowance form you get the higher rate. But apart from that I dont know if it helps with getting the nhs paid carer's (free care). Think if you look it up you might get some more imformation. Good luck Janexx

DS1500 also is a fast track system to process attendance allowance claims - good luck

Hi Linda

Sorry to hear about your mum's eye problems. Hubby had sore eyes for months, eye drops did little to help. The solution so far is to wash them with clean water several times a day, just use a clean cloth and gently remove all the "debris". In addition 2 to 3 times the same using weak tea. ( no milk or sugar added!). So far this has helped to keep the redness away. Good luck, take care, maddy

I always use warm soft soapy water withu little drop of baby oil added . It lessens the debris and gives lovely lashes at the same time . I also use cold tea bags to soothe .

A nurse told me about the oil when my mum had a stroke
