Dx August 2023 .Started triplet therapy Firmagon+Docetaxel+Nubeqa. Psa 2500 dropped to 0,4(August 2024) .All metastases shrinked. Now March 2025 Psa has rised to 8,1 .Blood tests yesterday,ct-skanning March 7. and specialist/urologist in 11. March. So I think situation can change rapidly and hope change to sorrow.I am in castration resistant stage and my cancer is very aggressive.I had very widely spread metastases all over my bones. And lymf nodes in my stomach area. They all looked good after triplet therapy. Specialist said excellent success... Well ...I think I wont take chemo(cabazitaxel) .Living with sideeffects 3-4 months and only get about 3-4 months more time via chemo.I will ask Xgeva and maybe Xofigo. And go to palliative Polyclinic/team. I already went to local funeral home on monday with my wife and arrenged all so it would be essier for her.. Maybe 6 months to year or so left. But thats life. We talk honestly about everything with my lovely wife.Everything is ready (paper works, money issues ,funeral etc).

I am not afraid.Just very sorrowful.

Could you switch to Abiraterone plus prednisolone?

Or at least add abiraterone plus prednisolone to Nubeqa?

Another reason to do chemo is it reduces bone pain.

Try chemotherapy again and then switch to Abiraterone plus prednisolone

sorry to hear. I recently read that they’re doing these trials on testosterone blockers for short term and reversing it with heavy does of testosterone and it seems to be working against these aggressive prostrate cancers. I would just try anything and go down fighting.

Sadly,, I am close to where you are. My oncologist gave me Xtandi back in January and it was worse than the cancer, but the cancer ignored it. I lost over 20 lbs and the doctor is giving me a short break to get better before trying something else. Xtandi tore up my stomach and made it hurt to eat. I don't know yet what that something else will be, but if it doesn't work, I probably don't have long either. I don't think I could survive chemo and don't really want to try.

Sad news from the happiest country in the world. I have extensive bone mets, 2017. Also abdominal lymph nodes at that time. Gleason score was 9. Told about a year and a half to two and a half. My PSA was no way near 2500. Did Lupron and Doxetaxel. And have been on Xtandi for 7 years now. Don't see where you have tried Xtandi or Abiraterone. Believe that is usual 2nd line of defense. I take Xtandi at bed time and have no issues. It's been 8 years this month and scans looked pretty good. My PSA is slowly rising now. I did Xgeva for 3 years. I do take one dose of Celebrex a day. Not a good drug if you have heart issues. Don't bury yourself while you are are still alive! I will be 75 this year. Not a doc, but I vote try Xtandi or Abi. (Still on Lupron (Eligard)).

I'm very sorry Petrig, I know that having everything worked out and a loving wife is of comfort to you as it would be for me. This world is a suffering world in many ways but it's a blessing you are not afraid. Blessed are those who mourn and I personally believe that there is a better life after this one and pray that you will be there, if it comes down to that.
