Trimix Problems

I'm hoping some of you guys out there can give me some good information. For background, I had a RP one year ago Wednesday. So far, so good, with stable psa and basically great continence (hooray!). The 3rd leg of the stool has been a bummer tho. I was having moderately severe ED issues prior to the procedure, and surgery resulted in 0% left sparing, 75% right sparing of my erectile nerves.

Using the Salt Lake Huntsman Center's men's health unit for rehab, I wasn't started on low dose trimix until October, with no therapy other than ed-5 ingestion until then. I achieved a 40% erection in office on 10 MLs and received a cheery prognosis. After 8 months, that's close to the best I've come up with, cycling thru low dose to 40 units, high dose to 40 units, and I'm now at 45 units of bimix because the injection and post "erection" pain was so great. To confuse matters, after finally discovering the penis pump, I had one blue steeler at 35 ml high dose trimix that almost sent me to the ER. I could never replicate it again and a few of my latest fairly high dose shots have created no response at all. Generally I get a fluctuating response of up to 80%, but never good enuf for penetration, except once several times ago. Confusing.

I'm despairing of this grim process but am scared of the finality of the implant process. I'm developing what feels like significant scar tissue on both side of my corpus (1/4"+ knobs), and even before that, injecting often required a lot of pressure and patience and pain. I'm worried that I'm doing the injection process incorrectly or missing the bullseye, but I'm doing it where and how they showed me. Burying the 5/8"needle, but sometimes it feels like the juice isn't going into the column and I get side swelling under the skin. The NP doesn't seem to really answer my concerns, I've been disappointed by what seems like the rote process and lack of individualized attention and hands on instruction in injecting. Feels like I'm on my own out here and would love some input from those of you that have gone thru this process. Something I'm missing or time to give it up? Recommends in the desert southwest (I'm in southern Utah)?

Are you standing after the injection? Massaging the fluid around?

Yes. Generally I can tell from the feeling within a few minutes if I'm going to get much of a result. Often get to just not quite a servicable erection and then have it drop off in 5 or 10 minutes to something much less. The unpredictability is what has me flummoxed. Has that been reported much by others?

If you are not happy with the instruction you are getting why not get some second opinions from some other docs that specialize in this particular problem.

I have low regard for Trimix and would never take it again. The swelling, despite everything recommended to mitigate it, the sometimes painful and inconsistent erections, the overall feel of it, everything.

That said, I know many have had good success with it, some I know personally.

If I remain in good health post treatment for an arbitrary time set in cooperation with my MO, I plan to get an implant without hesitation.

Proper instruction seems to be lacking for you. I had a nurse help me and could contact her with any issue's. They increased my dose by 5 MG until I hit a satisfactory erection. I ended up at triple strength 34mg. I inject 1 inch from the base just to the right side from the top of the penis. Then change to the left side on the next injection. I massage the site after the injection. This helps any bending and also any bulging. I am really happy with it and so is my wife. As the nurse said however , the desire must be there or it will not work.

My experience is very similar to yours. My urologist insisted that I start on the lowest possible formulation, which of course was worthless for me. Nothing was the least bit effective until I got to Trimix, but my response is like you---I do not get an erection capable of penetration. Like you, I have trouble finding help. The staff here (I'm with my second doc for this) doesn't have much patience with men who don't achieve early success. The NP does not believe any injection will work for me at this point. Like you, I had an experience where the needle slipped out of the target, and I had a lot of swelling and discomfort under the skin that required a couple of weeks to resolve. I saw the suggestion for an auto inject device, and while that might help prevent another messed up injection, it won't improve the erections. There is a product called Muse that you insert into the urethra, but I have never used it. It is very expensive, and I haven't heard any great reports about it. Best of luck to you, and please let me know if you come upon a good remedy.

My uro himself gave me my first shot with very definite instructions. I would definitely find another doctor willing to help you with this. Also, there are instructions on YouTube on how/where to give the shot properly. I do still find it a little inconsistent. But not to the extent you are experiencing. Good luck.
