I am 7 weeks out from surgery and all is pretty good. (3+4 Gleason, 8.8 PSA, 64 years old and excellent health). Catheter came out after 7 days. Incontinence issues not as bad as I anticipated. Diligent kegel practice. Also doing that pump thing so penis appears not to have lost any size/girth.
Was off my daily exercise regime (yoga) for 6 weeks and after a few classes this last week, I feel like I'm not too far from where I was when I went in for surgery. Still need a pad just to be safe!
The problem is ED meds (tadafil and viagra). Both cause rather unpleasant acid reflux. It's gotten to the point where I've stopped taking the meds.
I've masturbated and experienced "ejaculation" a number of times but the urine coming before and at climax is kinda weird and takes some getting used to. (No there yet....)
My question.... Can I expect to ever have a decent erection (if "10" was what it used to be, I'd be happy with an "8" but right now I feel like the best I can get is a 5 or 6.) Prior to surgery, I'd never needed ED meds (grateful) and if anything, getting hard was too easy. (More grateful!)
Anybody been down this road? I'd love to avoid the ED meds. They just mess with my appetite and exercise regime.

I've tried citrulline and arginine to increase nitric oxide. I think it helps a little. Perhaps take daily Prilosec to prevent the excess acid. One problem with ED meds is they may exacerbate incontinence (tadalafil is indicated for LUTS because it makes peeing easier) and may contribute to climacturia. You may be better off switching to "on demand" ED meds, along with your daily pumping. If you want a stronger erection, you may be able to attain that with trimix.
You are a font of wisdom and we're really lucky to have you on this platform. Thanks for your thoughtful reply. l will give Prilosec a try with Viagra. (I gave up that Tadafil weeks ago.)

As for "on demand" ED meds, would that be inline with my urologist's instruction to load up on Viagra (100mgs) 1 hour prior to intercourse? And Trimix (not both simultaneously, of course.)
Does daily pumping enhance/increase the chances of a stronger erection? I thought it was only for 'restoring' the penis to 'original' size.
And the amino acids will be first on the list b/c I'm inclined to do anything/everything homeopathic before jumping into medications....
My URO sent me to physical therapy that specialized in penile rehab. Sh put me on VAC daily and trimix every 5 days. At 3 months th URO told me he removed all my nerves. Got an implant. Have climatura also.
It works very well, but the nerves that cause the glans and the rest of my penis to fill out were removed also. Erections and filling are triggered by 2 separate nerve bundles. My sensation is down but this is all I have now. After RP my penis was a turtle. With the implant, flaccid is just 1/2 inch shorter than erect. Lots of junk to store away.
Seeing Urologist tomorrow and will discuss Trimix.

You won't get any erectile action from oral drugs, until your erectile nerves have recovered from surgery. Whatever your surgeon says about "sparing the nerves", it's a toss-up (maybe 50/50 odds, less IMHO) whether they will recover over the two years after surgery.
Tri-mix and bi-mix have a good chance of working. The "Yuck!" factor is large, until you get your first post-surgical erection. After that, the injection becomes an acceptable cost for real intercourse.
My experience is that an orgasm, with an erect penis, doesn't leak much (if any) urine.
I take a bunch of drugs for various reasons so I have found several drugs have given me acid reflux or at least a feeling of acidity in stomach (Viagra did, Cialis doesn't). Doctors advised to try taking them with food, and/or taking with Acidophilus. Both methods have worked well for me. As long as the drug instructions don't say "take on empty stomach" you should be o.k. doing that (I've not seen that warning on any of the ED meds). I've also learned that for me "with food" means «in the middle of the meal». One of my drugs is an acid itself so really triggers the GERD, so I find making sure I take it mid-meal then cover it with more food makes the problem go away. Experiment, see what works for you.
Have patience as it takes time to recover your erections. I am 18 months out and just starting to get a bit of a boner on viagra. Trimix is a blessing,
I had my RP back in June '18. I was 63, G9 advanced PCa. From clear scans Dx'd localized. My surgeon specializes in nerve-sparing, however since PCa was found extra-capsular 2/3 of right side never bundle was removed. I am now almost 21 mos. post RP. Following are my experiences to date, as an offering that they can assist you (or others?) in recovery choices.
Continence issues (requiring pads) continued for over half a year. Prior to surgery, I never had any issues in this respect. Then it got better to where I seldom need pads (occasional drip). This change seemed to come about sudden. God bless kegel exercises! I discovered you can overdo them. Make it a daily routine, but don't be excessive as I initially did.
I too suffered ED, say a 5 or 6 out of a 10. Like you, I had no issues at all prior to surgery. Initially, the thought of using a pump or taking enhancement pills was uncomfortable to me. Instead I chose to give my body time to heal on its' own. I did however continue kegels.
After a year's time (post RP) - I began experimenting with sildenafil (viagra). Prescription was 1 to 5 pills per use (20mg each). I tried all dosages several times over. I found that more than 2 pills (40mg) did not make any more of a difference in stimulation. I also purchased a pump, and have found it useful. With both, I can achieve enough strength for intercourse.
The sildenafil had a negative side-effect. It provided the blood rush to my penis, but gave me a pressure headache as well. It was uncomfortable. Enough that I would decide each time if the tradeoff was worth the undertaking. I found that 2 aspirin (Excedrin) after would relieve the headache. I next found that 1 Excedrin taken at the same time I take sildenafil worked!

exercise if possible, to increase blood flow. kegel exercises, again, if possible. I've used viagra and cialis, had exact same effect! as Yohimbe - an herbal supplement. Ask you doctor before trying it. The pharmaceuticals are supposed to lower b/p but spiked mine.
