Hi , somebody asked me to give more details on my situation . So , I am French living on the Riviera near Monaco . After rising PSA , 3,7 in 2018 , 4,8 in 2019 and 5,3 in January 2020 , on this last index , I decided by myself to see an Urologist at Monaco Princess Grace Hospital . Immediate RMI , Pirads4 , 11 mm oval lesion left lobe posterolateral touching the capsule without external invasion . Biopsy 2 cores among 15 were positive , one 3 mm , the other 4mm with 7% of score 4 in it . I was then proposed surgery with lymphnodes removal which meant 4 days in hospital and catheter for One Week , was then reassured that I would not face incontinence but the left NVB would probably be affected . I chose RT plus chemical castration by 2 injections over six months of Enantone , Takeda Japanese Lab . My RT consisted of 38 sessions of 2Gy on Prostate , prealably contoured by three gold grains and my SV received 54 Gy . I asked why the SV also ? The RO responded that I had intermediate risk cancer and the SV could be affected in theory . My RT finished end of this June and my second and last injection end of July . In conclusion I felt Nothing with RT except some frequent urination which have disappeared and with the ADT , very few side effects , ED certainly , more wrinkles on my skin ( legs , arms ) no weight gain at all , and my PSA is currently at 0,06 and my Testosterone 0,7 nmol/L . I feel well , was treated well by Princess Grace Hospital ( I have seen the Princess several times when I was at school in Monaco in the early 60's when she usually visited our classrooms every year with Prince Rainier ) Would like to compare my situation , my problem would be ED and certainly no semen with ejaculation . Hello, I am 73 y.o. And I had 6 biopsies out of 12 positive G4+3=7 Grade 3 very aggressif and average 80% (70% to 100%) of the 6 cores that were an average of 17 mm.

My PSA was 20.4 pre-treatment. I had 2 injection of Lupron Depot 22.5 mg/12 weeks (from May 31st 2020 + August 24th) and VMAT Hypofractionnated RT of 20 X 3 Gy for 60 Gy (équivalent à 78 Gy conventionnel) on a very wide margin, about most of the pelvic bed (139.37 cc while the prostate is 45.83 cc and my SV are 4.50cc) in all the directions (which cover the whole SV) and half of the blader with 58 Gy from June 8th 2020 to July 07th 2020..

I am risk Intermediate unfavourable.

My last test showed PSA 0.18 ug/L and testosterone <0.2nmol/L or <5.7684ng/dL.

I will have another test on the September 15 and a meeting with my RO on the 28.

I am happy even if the second injection of Lupron Depot hit me very hard. Hot flashes very intense and numerous, changes of mood and agressivity wich did not last..

But it is temporarily and I hàve 1 daughter and 4 sons and 2 grandchildren that I want to see growing up so I will put on with every treatments.

Your are right. Here in Montreal, Canada, you woukd have been put on Active Surveillance.

BTW how is the temperature in France. Et je peux parler français.

Bizarre, ton APS est plus bas et la testostérone est plus haute.

Question au sujet le disfonction érectille, je ne peux pas te donner d'avis étant sexuellement non-actif depuis plusieurs années. Mais certains matins je me lève avec une érection qui ne sont pas érotique. D'après mon RO les effets de l'ADT devrait durer un autre 3 mois, soit un total de 24 semaines à partir de ta dernière injection.

Le volume de l'éjaculation va diminuer avec le temps. Ne pas oublier que les effets primaires et secondaires prennent des années à agir.

Moi je me considère chanceux. Mon RO m'a donné un documents de 7 pages de la planification de ma VMAT et ainsi j'ai pu savoir combien de Gy chaque partie de mon pelvis a été irradiées, ainsi que leurs volumes.
