Urethral Strictures...TURP/PCa/Proton

Does anyone in our group have a experience with recurring urethral strictures which are a result of a TURP last August and proton beam radiation in the winter? I am getting these strictures now for third time since this all began. For past 4 months I have used catheter once a week for dilation purposes, a practice which I’m told should work over 95% of time, mine has not. Really need some ideas.... thanks.

Is it from scar tissue? A collagenase (Xiaflex) injected into the scar tissue may clear it up.

Thanks for quick reply. We presume it’s from scar tissue. I will talk to my uro next Wednesday prior to cysto and likely cutting more strictures. I do have some calcification as well so stones could be getting larger also causing problems. Won’t know until he takes a look. I will discuss the xiaflex with him.

Some guys are prone to forming scar tissue/collagen. The more they try to cut it away, the more it grows back - bigger than before. It may be possible to break that cycle by chemically dissolving it with a collagenase. Xiaflex was designed to dissolve collagen in the penis that causes Peyronie's. Hopefully, it or another collagenase will work for you.

How does this get injected and exactly where🙉. Yes it does seem that the more they’re cut the more they come back. Thanks

It gets injected at the site of the scar tissue. I'm guessing it can be done through or in conjunction with the cystoscope, but I really don't know.

Thanks again. I’ll pass along to uro.. appreciate the help....

I didn't have such a great result with a xiaflex injection for peyronies plaques -- it landed me in the ER with retention. After reading a post here by Patrick related to dupuytren's I ordered iodoral 12.5 mg. This is an iodine/iodize supplement. I took 1/2 pills for a while. Within a matter of weeks my peyronies bend was cured.

Thanks for the info. Not sure if Xiaflex will work but will be discussing with my uro this week. Appreciate the help.

Expect the uro to be dismissive of iodine. After my iodine experience I confronted my uro -- the one who had done the failed xiaflex injection -- regarding my miracle peyronies cure. He chalked it up to a 'coincidence'. I told him that after more than a year and a half suffering from peyronies after the failed xiaflex it was quite a coincidence that the problem resolved after a few weeks of taking iodoral. He then muttered something about 'no clinical trial' for iodoral. Expect the same from your uro. Just get the iodoral and try it. It is available on Amazon for $35 or so. What have you got to lose, other than an iodine deficiency? Just don't over-do it.

Will check it out but my issues sound different than yours? Mine are result of TURP then radiation for PCa.

The cause of my peyronies was a mystery. The iodine cure isn't -- to me anyway. The peyronies plaques are, or were, scar tissue. Do some research on Dr Brownstein and iodine. If nothing else, the iodoral is good to have in case of, God forbid, nuclear fallout. By the way, I had TURP years before my prostate cancer diagnosis. I suffered no ill effects from that surgery. Everyone is different. It might also help to take L-arginine and Acetyl L-carnitine. I was taking those in addition to the iodine. Helps with circulation 'down there'.
