The diseases of excess - Obesity, metabolic syndrome and hyperinsulinaemia seem to make you more susceptible - and also old age and underlying medical conditions.
I think that it spreads in hot dry countries, but not so much in hot, humid countries.
There are countries in South Asia and Central Africa with low GDP, and high average temperature and humidity. If they have little obesity and metabolic disease, and few of them live to a great age, will they weather the outbreak (without intervention)?
If this was an epidemic in one country - the world would help... and it might be that we will try to help when we are over the worst.

Yes - the western SAD high-(refined)-carbohydrate diet causes persistent high blood glucose, and the body reacts by producing more insulin, which converts the blood sugar into fat (and tends to cause diabetes). Ultimately, the insulin fails to control blood glucose, and the result is diabetes, which is defined in terms of blood glucose.
Diabetes tends not to get diagnosed until the insulin level has been high for decades, so hyperinsulinemia is a better, earlier indication.
It's also what happens when the pancreas has too many islets and then needs to be mostly removed (islets and pancreas). This can also cause the person to become Type 1 after surgery. The surgery is a 1% chance of survival.
