MRI results from having groin, thigh, pain

well after over week and half of severe groin pain, and upper front thigh pain, only able to sleep a couple of hrs each night, after icing, and heat, rheumy ordered MRI on hip, and back exray, saw him yesterday, and he was rather vague about results, except to say, showed tendonitis, small tear, and hip bursitis, the severity of pain has subsided somewhat, for now,but wanted to know what happened to cause all this, basically said, degenerative, age, wear and tear etc, said anyone over 30 would not have a normal MRI, !!? said could be prednisone use for length of time I have been on, over 6 yrs now, I have now tapered to 4 mgs, been there for about 6 weeks now, he said to continue taper, he handed me copy of MRI report to give to my physiotherapist, and chiro, suggested could give me cortisone shot i"n hip if required. My physio therapist I had thought was quite good, had been going to her for last couple off mos, but this latest episode of pain, started up seems like after last visit, when she did "dry needling" along with other methods, I was going to her as suggested by rheumy for stiff tight glutes, and hamstrings, back issues etc, but havent been since all these other issues happened 2 weeks ago now. When I read the MRI report , there are many more comments, all in medical terms I really don,t understand, one being a cyst was seen "rectus femoris tendon " paralabral cyst measuring 2.8 x 1.2 xx 3.5 cm ??? which he never even mentioned., This is the 2nd rheumy I have had now, this one since April, and truthfully, am not that happy with his help, he stood up as if wanting to end my appt, while I was still asking questions, not sure if should be looking for another one, as far as pain goes, all he said was take acetominiphen, which of course has not been very helpful all along , just relaying my experience, little venting, kind of feel as many of my friends have felt, as you age, some dr,s don,t seem to want to spend much time or explaining to you, just reaching out for any thoughts thank you

I would be looking for a new rheumy, for sure! How rude and unprofessional to dismiss you before listening to your questions! I haven't heard of the conditions you mention but a couple of links below may help you decide what you should be doing (or not doing) next.

I hope your issues are resolved with someone more caring.

l have been on prednisone for 40 months.

Recently had similar hip bursitis. I would definitely recommend an injection. A knowledgeable orthopedist is what you want Methylprednisolone can be a much more effective steroid than prednisone. Also please have your thyroid tested. TSH, Free T3 and T4. Often bursitis and carpel tunnel are related to hypothyroidism.

"Methylprednisolone can be a much more effective steroid than prednisone"

Not necessarily - it depends on the person. It made me really unwell and I needed a far higher dose to get symptom relief - which took several hours to take effect. I also had steroid side effects I have never had with prednisolone or prednisone.

You are right in mentioning thyroid problems - it should always be a rule-out in the differential diagnosis of PMR since PMR is a diagnosis of exclusion. But it isn't only hypothyroidism - it can happen in thyrotoxicosis.

well I see rheumy again this Monday, a follow up and perhaps as he suggested cortisone shot in hip, the hip has settled a bit , but still get slight ache in front thigh, and twinges in back and groin, so Im really at a loss as to what to do, wasnt clear exactly where the small tear in tendon is, and having given me a copy of MRI report, among a lot of other comments, talks about a cyst? which as I said he didn,t even mention, only saw it when I read the report, which is full of medical terminology I don,t understand, I am seeing my GP tomorrow for flu shot, and will ask him a few things, as a report was sent to him, don,t know if he will be willing to discuss though

I had very similar problems eventually diagnosed as Trochanteric pain syndrome. It also causes pain in the groin and is so painful and disabling. If you have a cortisone injection I highly recommend you go to someone who does them under ultrasound or X-ray guidance. I have had them at a hip dr and a pain clinic. The pain clinic has been the most helpful with these musculoskeletal problems but the waiting lists are long in the UK. The GP and rheumatologist just seem to stick the injection anywhere in the near vicinity. Hope you feel better soon.

My rheumy does s physical examination of the area to find the spot that elicits the loudest shriek from me! But he unerringly identifies the right spot first time and the injection works very quickly.

my lovely GP does the same. Right spot first shot.

Methylprednisolone gave me the shakes everyday at 4-5 pm. I eventually switched to prednisone and no longer experience the shakes but I do experience fatigue and acheyness sometimes also around that time.

I had the most superb black beard! Having been totally white since I was 45, no idea what that was about! I gained a lot of pred-weight in the usual places. 20mg didn't cut it. I switched to prednisone and started with 15mg again - and the relief was like that first day on prednisolone years before. The beard went, the weight went - even at 15mg. And I felt SO much better.

I still deal with constipation as a side effect of either methylprednisolone or prednisone, which from my understanding, diarrhea is more the norm. But I never professed that I was normal, so there's that!

Goes to show how different types of corticosteroids effect each of us differently.

Wow. That sounds a lot to cope with. I've only had pmr for 2 years now and seem to be coping reasonably well. I started on 20mgs and like you am down to 4mgs now.I had a torn muscle some months ago and was recommended a physio but all she did ws to tell me to buy a special hard ball and stand with my back to a wall putting the ball inbetween the problem muscle and the wall then rotating it with pressure. I was lucky that this worked after 3 days and if i feel it coming back then out comes the ball. I have forgotten the name of it but I'll look it up and let you know.

I was advised to walk as much as i could but it wasnt easy. However, i still walk when i can and know it is beneficial.

You have other problems which i am sorry i know nothing about but i do know that this ball works for me and the walking helps my tightening tendons.

I wish you well and hope things improve for you. X

I was just diagnosed with hip bursitis and sciatica in same leg. Very painful. Had hip injected with steroid 2 days ago and having physical therapy 3 days a week So far very little improvement but I believe bursitis takes a long time to resolve. My Dr rushes in and out and barely get to exchange good info with him.ive been on prednisone about 2 years. Hard to find a good rheumatologist in the rural area I live.Hang in there and I hope your GP helps you with the MRI results.
