Firstly, sorry for my bad english, it's not my native language.
I'm 34, living in Canada. 6 months ago, i had some very light pain in epdidyme area and scrotum.
Then it disappeared a little and in august, i had a huge flare-up with many more symptoms.
As today, i have pain everyday :
- scrotum electric shock on both scrotums.
- Groin and perineum spasms (a lot when i'm driving)
- Some light pain in hamstring area
- But the most disturbing thing is everytime i walk more than 200meters, i have a strong pressure in the rectum. EVERYTIME. I have to lay down 1h to the pressure to disappears
Of course i can't sit more than 30mn.
I've seen 4 doctors, PT, uro etc etc.
I feel I am becoming depressed...there is no doctors to help me in Canada, i just want to know what is going on.
I don't understand why i have bilateral pain (both sides). If someone can explain to me. I thought when it was related to nerves it was only one side.
Do you think it can be piriformis syndrome or pudendal ? How can i do to see if it's the piriform ?
I hope this is this one and not the pudendal which seems very hard to cure...
Also have you already heard about this symptom during a walk ? It seems i'm the only one who have this kind of disturbance.
Thanks a lot for your replies...

Yona, being a female, I don't have the same symptoms you have, but since I self-diagnosed myself with Pudental this year, I experience the pain on both sides (everywhere down there actually), though I have read many posts stating they only have it on one side. I'm still looking for a solution, will be attempting PT in the near future.
Hi Cheryl! Are you sure you have pudendal? I've read that piriformis syndrom can starts pudendal-like symptoms. If your both piriformis muscles are tight/loose, they will compress the sciatic nerve and the pudendal nerve. So the pudendal nerve is maybe affected, but this is not because of a classic pudendal entraptment. Heal the piriformis syndrom can maybe cure the pudendal symptoms.
It can explain why the pain is bilateral in my case. Also i have more and more sciatic-like pain in my legs. I've tried to diagnose myself with some specific gentle stretches to test the piriform muscles; It appears that after 10mn my glute and my hamstrings was on fire after that.
I see my PT tomorrow, i will ask him to check all of that and see if my pudendal symptoms can come from piriforms muscles.
Believe me, I would prefer it be a piriformis issue, I'll start researching. I've had piri pain in the passed, maybe its just gotten worse and spread to other areas. Good luck tomorrow.
it’s not pyriformis or a primary pudendal nerve entrapment. Deterioration in response to posture and excessive raisins the possibility that an occult prolapse has been missed?
Very unlikely. Have anus checked by my uro and my Md. Never had constipation this year, and no symtoms of rectocele/prolapse. I've seen my PT today, he continues to think the real cause is my back (middle of my back, which is painful to touch after my fall in the stairs 10months ago). I'm going to do some news stretches.
Where in Canada do you live? I am also in Canada. I've been having pelvic floor issues for 3 years, but no diagnosis yet. Maybe a good Pelvic Physio would be helpful. Someone who is also an Orthopaedic Physio. If you go to the website pelvichealthsolutions.com you can probably find one. My Physio is more knowledgeable than the Specialists I have seen. Good Luck.
Hi, i live in Montreal. It's very complicated to find the specialist who will give you a diagnostic. I'm a french guy, and i lived in Nantes, which host the best clinical team. Too bad i can't go there to have a real treatment.
What a pity. I hear the team in Nantes is the best in the world. You have probably seen a Urologist already. My suggestion would be a Pelvic floor PT. They know other doctors and other people who have been treated effectively. Bonne Chance!
