I am so sick of being in pain. Have Pudendal Neuralgia burning in left of vulva, Clitoris and into crease of leg, Sciatica, burning foot, and piriformis pain made worse by dry needling and stomach pain and cramping. Have been in pain for 7 months. My piriformis is so painful and worried dry needling has damaged the nerves. Was too nervous to try medication properly only took 1 cymbalta and 1 amtriptylline and didn't like the effect, have also been give lyrica 25mg but haven't tried it yet. Have been to Pelvic physio but not sure if it helps much. My pain is much worse when I have my period. What else can I do?

Sorry you are in pain. I also have burning in vulva and anal area all left sided for the most part. This started in January of this year. I have not been diagnosed with PN but sounds like it. I have been taking Lyrica 300mg for 5 weeks now and Fluxotine. I just got a compound RX of Amitriptyline and Baclofen yesterday to try applying. Maybe the Lyrica is helping some, not enough, but everyone is different. How did your pain start? Did it come on slowly? This is not easy to go through. Lots of prayers!
Hi Louise, I can tell you with confidence that dry needling is a waste of time. You can accomplish the same goal with a racquet ball or tennis ball. As for medication for true PN, you need to be on 50-75mg of amitryptline and up to 400mg on Lyrica to gain max effect. Yes, it will at you loopy in the beginning, but you have to push through it. You can do it!
How long do you have to take medication for?
If it’s pudendal neuralgia, you’ll need it indefinitely.
I’d caution against using Lyrica. It’s a dangerous drug. Instead use Compounded vaginal suppositories containing 10mg. Valium and Lidocaine. Insert two at bedtime. If you’re lucky, everything may calm down.
Just wondering? Do you have any mesh or metal hardware inside your pelvis?
Hi Louise. Dry needling by a properly trained physical therapist can be very effective. Too many chiros, massotherapists etc, “learn” needling in a weekend & aren’t properly trained. That said, I think you should take the meds and give them a chance. That way you’ll know for sure whether they help. I’m sorry you’re in pain.
Can dry needling make the nerve damage worse as I didn't have pain in piriformis before having it done now it won't go away and my leg feels numb

Your situation sounds a lot like mine. Amytryptyline has helped me a lot. I don't like taking it but the side effects calmed down after a few weeks and it stops me feeling pain. It's still there but I experience it more as a mild sensation than a pain.
I feel much better now it seems to have calmed down on its own thankfully I am not in bad pain anymore just sometimes mild symptoms and sensitivity.
