Hip Labral Tears can cause vulva and pelvic pain, and mimic Vulvodynia and PN

Hi

I have posted something here about this before and I've seen a few other post regarding this subject but I thought I would re-post it just in the hope that it helps someone. I'm hoping that if anyone here has any sort of hip pain, arthritis or has done a lot of sports this might offer some hope or even a solution. If anyone has a diagnosis for a Labral tear or gets a diagnosis for one in the future it would be interesting to know about it and if treatment for it helps their pelvic issues at all.

Since going to a physio for another issue (neck related headaches) I'm becoming more and more sure that hip damage adds to some of my problems. I've always had hyper-mobile joints and a tilted, twisted pelvis so I'm pretty use to my joints aching, mainly my right hip and right shoulder. I asked the physio if I could do anything about it and the first time the physio tried to realign my pelvis the pain the next day between my legs had really intensified (she did warn me that would probably happen) 2 days later it had died down a lot.

Causes of labral tears include trauma, impingement and degeneration. Symptoms of labral tears include hip or groin pain, buttock pain, clicking, locking, loss of range of motion, and giving way. Others may have pain in the vulvovaginal area or testicles/scrotum and have pain with intercourse. Hip labral tear symptoms can mimic or be related to vulvodynia, pelvic floor muscle dysfunction, or pudendal neuralgia.

hello purple tea (love your name) can you tell me with your condition did you feel like you had an object in the vagina ? Hello, No I don't have a feeling like there is an object in my vagina. I've seen this a few times on this board though.

Pizon, did you ever find the cause of your issue? I have the same and the doctors act like I am nuts.

Hi Fiona well since my last post regarding this issue I had to have 2 of the permanent stiches (from the bladder lift) removed because they had eroded thru the vaginal wall..that dr. sent me to a pelvic pain dr. who specializes in pelvic nerve pain....my pain is better and I can sit for longer periods of time however I have stopped going to the nerve dr. (Richard Marvel in Annaplios MD) due 2 insurance costs I can no longer afforded him I am now taking cbd oil and working with an herbalist....taking valerian root and hops as well.....if you can get to a pelvic pain specialist that would be where I would advise you to start...I still have the feeling of a ball but it isn't as bad...the herbalist believes the dr that did the bladder lift twisted a sensory nerve and when the new dr. removed the stiches he did root around and cleaned out the areas where the stiches were that he had to remove....best of luck to you it was horrible when the dr.'s didn't know what was wrong with me please keep me posted I will keep you in my prayers.

HI PurpleTea: thanks for your input. I've been suffering from 'pudendal neuralgia' which does make my hip go out because of the way I walk and what they call 'guarding' the area of pain. I see a chiropractor every 6 weeks to get re-aligned. It really does help.

I'm glad you've found something that helps. Have you tried physio? Physiotherapists often give homework to teach exercises so that you can treat yourself in the week. I've found that the physio I see does more than when I went to chiro and she also realigns my pelvis.

Could a labral tear also cause the same rectal symptoms as PN, ie, burning, fullness, difficulty eliminating etc?

I have a left sided labral tear which gave me severe groin pain. This pain followed an osteopathy session. I don’t know if the tear was already there or if the practitioner caused it. All I know is that afterwards the whole of my pelvis was in pain and I heard a snapping sound as I walked and raised my left leg which wasn’t there before. Then all the PN related symptoms began on the left side. My life finished that day 😥

A 3T MRI showed the tear. My orthopaedic consultant had never heard of a tear giving symptoms of PN. He hadn’t even heard of PN. So, I don’t know whether it is the nerve or tear which is causing pain in the rectum, lady bits and urethra 😞

I have been suffering with pelvic floor pain, radiates to pubis, groin and vulva. I was sent to physical therapy for levitate ani syndrome but the stretches seemed to worsen my symptoms at times. An MRI shows a labral tear and I think it is the primary source of injury with all of the other pains referred from this. I think all of the nerves and musculoskeletal system in the pelvic region can have associated pains with inflammation. Certainly I will follow to heal the labral area but I have definitely found pain relief from 1) mild stretching2) anti inflammatory supplements( not NSAIDs ) like fish oil, turmeric, bromelian, glucosamine, CBD oil 3) meditation for stress and muscle relaxation 4) heat/ warmth 5) low dose amytriptaline at night 6) anti inflammatory diet ( gluten free) Hope this helps, I know it’s frustrating and sad to have chronic pain, I feel my life is limited or on hold while I try to heal. I am humbled and trying to bear this with grace and hope and I pray you find help and healing also.

Thank you for your reply Hopenwait. What’s your plan of action regarding the labral tear (are you in the UK)? My orthopaedic consultant offered a steroid injection which I declined at the time because I was struggling with terrible side effects from Amitriptyline. My body couldn’t take anymore. I was completely broken.

Can a labral tear affect the rectum because I note you mentioned levator ani?

My research does suggest that labral tear will affect pelvic floor muscles , this would include levator. Perhaps a “ tensing” or protective hypermuscle flexing we do unconsciously related to this injury. Very important to see physical therapist or if finances prohibit then research home stretches such as hamstring stretches to relieve muscle tightness. I am sorry amytryptaline did not help. St Johns wort May have similar properties in low dose but obviously though pain relief is important , getting and curing the source of pain is preferred. I am going to try physical rehab versus surgery right now I think because of my age (57) and I must go back to work. Labral surgery is not as effective in some to cure problem depending on your age, severity of tear, success of other treatment, and your current pain level. If your consultant did not know that the symptoms of PN and labral tear can affect similar symptom areas then ? It is interrelated anatomically and neurologically in that region so seems common sense to me. The most effective in my relief has been gentle stretching, calming the F@$& down lol, supplements to help inflammation, and prayer or meditation to breath thru pain , conscious muscle relaxation of pelvic floor. Research Dr Susan Gronsky in-line was helpful. Good luck.

Thank you for your reply. You know far more than so called specialists. Please keep us updated on your progress. Have you researched David McCoid’s DCT program? I only ask because you mention stretches. I will research Dr Susan Gronsky.

As its very difficult

Only after research of 13 years on and of even after seeing so many Drs

I believe I have pn and I am in the uk and there are only a handful of Drs who understand this condition

Pudendal hope website was where I found out I pray you all get better as I get sever pains in the private areas

And I take

Morphine

Tramadols

Paracetamols

Pregabalin

And also I had a kidney transplant so have a lot on but I guess we keep going

A 3T MRI (no contrast dye) clearly showed my labral tear which they catagorised as being small.

Vulvodynia, pudendal neuralgia - how what why. Doesn’t make any sense!!
