I was told after a nerve study on my leg that I have “ old nerve damage” from a herniated disc. However, I also have pernicious anemia and MTHFR (2 SNPS). I take B12 injections about once a week and I take 400 mcgs of methyl folate daily. Is there anything else I can do?

Just keep up regular b12 injections 💉
It's possible but very very slow .
Some damage never fully recovers but its really unknown I wax told by a neurologist .
Make sure other vits and minerals are in balance.
Eat as well ad you cxn and drink water.
Peripheral nerves can benefit from B12. Although unexpected, my visual disturbance corrected after about 2-3 years of taking lots of B12 sublingual with the injections. I’m now down to 1 injection every 8-12 weeks. 10 weeks is probably my limit but, no return of nerve damage here.
This is no quick fix but hopefully as the years pass you will look back and realise the distance you have travelled. However, I’m not sure if damage from a herniated disc is repairable. I have had a problem in the past where a nerve that feeds my left hand thumb and adjacent fingers was being crushed in the neck. B12 didn’t appear to prevent or repair but neck traction did wonders.
