If you are in the UK then recommended treatment in terms of maintenance doses is 1000mcg hydroxocobalamin injected IM every 8-12 weeks.Regimes are different in other countries, particularly where cyanocobalamin is used (would normally be maintenance every month.

Please note that this frequency doesn't suit everyone.

Some countries do use high dose oral as an alternative and recommendation would be at least 1000mcg daily.

The British National Formulatory - that NICE use says this:

HYDROXOCOBALAMIN

Prophylaxis of macrocytic anaemias associated with vitamin B12 deficiency

By intramuscular injection

For Adult

1 mg every 2–3 months.

Pernicious anaemia and other macrocytic anaemias without neurological involvement

By intramuscular injection

For Adult

Initially 1 mg 3 times a week for 2 weeks, then 1 mg every 2–3 months.

Pernicious anaemia and other macrocytic anaemias with neurological involvement

By intramuscular injection

For Adult

Initially 1 mg once daily on alternate days until no further improvement, then 1 mg every 2 months. I will add that this means exactly what it says and it may be many months or even years.
