think they might mean causal!
This was interesting too, never seen this low indicated before
'Anti-intrinsic factor antibodies should be tested if pernicious anaemia is suspected regardless of B12 level- may be found in up to 35% of cases of pernicious anaemia. Life-long replacement therapy is indicated.'

Do you have any thoughts on how 'optimised' should be interpreted in the context of gp decision making?1.6.12 If the symptoms of the person having intramuscular replacement have not improved or they have new symptoms of vitamin B12 deficiency:
• ensure the frequency and dose of vitamin B12 injections are optimised
and
10 • agree a date for reassessment of the person’s symptoms.
That's a difficult one to answer as it all depends on the Gps knowledge of how to treat and how flexible he/she is with regard to treatment !
