Hi , i am diagnosed with PA 3 years ago and i am on regular methylcobalamin 500 mcg injections . Initially i have done an ultrasound and my liver was normal. After 3 months of injections i had FATTY DIFFUSED LIVER in the ultrasound.. And again today after 2.5 years i did an unltrasound and it showed GRADE 2 FATTY LIVER. i dont drink alcohol.. all including doctors are saying that my regular b12 injections(alternate day) are the reason for this.. Not sure what i should do as the doctor is saying that it will progress to serious condition like chirrosis.. Any advice will be helpful

It's got nothing to do with taking B12.
Go see a real doctor. One who has heard of non-alcoholic fatty liver disease - nhs.uk/conditions/non-alcoh...
One of the few papers which mentions both B12 and non-alcoholic fatty liver disease ends:
The serum vitamin B12 levels were significantly lower in the patients with NAFLD than in those of the control group; however, these still remain in the reference range. Consequently, low vitamin B12 levels may be associated with NAFLD especially in grade 2 to grade 3 hepatosteatosis.

You should have had some thyroid tests - among others.
Hello helvella , 2018 December scan showed liver was perfectly fine . So due to my neurological problems and many others diagnosed with PA. Started injections every alternate day. 6 months later ultra sound showed little fatty diffused liver. After 2.5 years it progressed to grade 2. I see the same pattern with triglycerides .. I don’t drink .. I developed NAFLD only after injections (this might not be the cause) am I missing some other vitamins due to increased b12 ?
I don't know of any particular depletion due to B12.
Given the number here who inject very frequently, I rather suspect that a simple "lots of B12 injections cause NAFLD" is simply not tenable!
Just remember that thyroid hormones are very much associated with triglycerides and cholesterol. It is not unusual for people who are hypothyroid to be offered statins, diet advice, etc., only for the issues to resolve when thyroid issues are identified and addressed.

TSH of 3.11 is higher than mine was when I started to get symptoms!
We rarely see Total T3 - but in itself doesn't look too bad. But it can be affected by carrier proteins.
Sorry helvella i did not understand. Do I have thyroid issues?. Should I have to check for antibodies as well this time? I feel I may have it since I have PA

They don't look awful - but the total t4 and T3 are not exactly high. And TSH is on the high end for healthy people.
I suggest you re-post over on Thyroid UK forum.
In the UK we almost always see Free T4 and Free T3 tests. The total tests have a tendency to reflect things like how much binding protein you have.
