Sorry i dont know about insurance for sure as i dont have any as i dont work due to this condition.
I have called cvs (nationwide US pharmacy) and the do offer 10ml vials of hydrox for $43, if memory serves, but a prescription is necessary.
I don't really know either. For one thing, every insurance company makes its own decisions about what treatments to cover and those coverages vary based on the specific policy you have.
For example, my company switched insurance carriers last year and my new policy didn't include coverage for my injectable cyanocobalamin prescription. It did include limited coverage for a health provider to administer an injection for me, but it would not have covered all of the injections I need (not to mention what a monumental hassle it would be to have to go to a doctor office every week to get one). In the end I had to ask my company's benefits manager to work something out with the new insurance carrier (which they did only because it is more cost effective for me to administer my own injections).
So the first thing to do is read your policy to find out what is covered. If it isn't covered, you can talk to your benefits manager or to the insurance company directly. There's no guarantee that they will grant an exception, but you might as well ask.
I did want to mention one other thing; unless you are prescribed to take 10ml of B12 in one month, you will probably not be able to obtain a 10ml vial of injectable B12 from a US pharmacy. They used to sell vials that size, but there's been a crackdown on only supplying one month of medication at a time. Instead, most pharmacies carry tiny little 1ml vials. I'm prescribed weekly shots, so I get four 1ml vials every four weeks now instead of one 10ml vial that could last me a couple of months like I used to be able to get.
