Hi all

Hoping you can help with thyroid results

Tsh 0.12 mu/l (0.35 - 5.5) low

T4 21.2 pmol/l (10.0 - 19.8) high

Not feeling too good also have PA ANDgoing through menopause !

Hi ollie7horse,

I am sorry I do not know much about thyroid, your results do not look right.

Your results look like Hyperthyroidism = Overactivity of the thyroid gland, most commonly due to Graves disease.

You need further tests. Ask your doctor to order Free T3 and thyroid antibodies. You are slightly hyperthyroid but you need to know if you are just hyperthyroid or if you have the antibodies for Graves Disease which is autoimmune. Don't let this go, get after your doctor because hyperthyroidism can increase the heart rate and can cause other symptoms, such as loss of muscle.

I have been on 125 mcg thyroxine for 28 yrs hypothyroidism pernicious anemia for 5yrs. my thyroid has been up and down since pa diagnosis. I self inject b12 twice a wseek

B12 treatment can mean you need to reduce thyroid meds..perhaps that is the problem?

Yes marre i was thinking the same doc has knocked down to 100 mg

I am feeling shaky, very tearful knackered (always)! And coming over really hot

All sounds like hyper to me, but am no expert! Hopefully you will improve with the reduction of thyroid meds soon, believe it can take some time, Marre.

Yes and the other way round, as in hyper until the thyroid more or less burns out to become hypo.

I would be wary of reducing your thyroxine as most people only feel well with a lower or suppressed TSH. My last test result was TSH of 0.01, with a fT4 of 22.02 and I am in no way hyperthyroid.

'Understanding Thyroid Disorders', written by Dr Toft, gives good advice and it is published in association with the BMJ. An extract : "

"Typical results would be a fT4 of 24 pmol/l or TT4 of 140 nmol/l, and a TSH of 0.2 mU/l. In some patients, a sense of well bring is achieved only when fT4 or TT4 is raised, for example 30 pmol/l or 170 nmol/1 and TSH low or undetectable. In this circumstance, it is essential that the T3 level in the blood is unequivocally normal in order to avoid hyperthyroidism."

You may have a T4-T3 conversion problem, but as Polaris says, I would head over to the TUK forum here on HU. The admins are very helpful and will interpret your bloods better than I can.

How are your levels of Ferritin - Folate - Iron - VitD ? All need to be good in range for the T4 you are taking to convert into T3. That is the most active thyroid hormone. Have you been tested for Hashimotos - which is the auto-immune version of thyroid illness ? When you suffer with this you can swing from being Hyper to Hypo as the anti-bodies attack and eventually destroy the activity of the thyroid...

The anti-bodies that need testing are Anti-TPO and Anti-Tg...

Are you taking a good B Complex with the B12 injections - to keep all the B's in balance. One with a good level of Folic Acid or Methyfolate. FA works with the B12 in the body and helps to keep it in the body longer I have read.....

Serum ferritin 57.1 ug/L (20 - 300)

Serum Vit B12 - unsuitable due to Haemolysis ( I self inject so not sure what this means but my b12 would be really high)

Also highlighted but doc didn't mention ( always come back the same in all my blood tests) was

Eosinophil count 0.6 10*9/L (0 -0.4) high

My cholesterol came back high as well (never realised underactive thyroid could contribute) it was 5.8 not too high and never had it before in the 28 yrs of being under active.

Are you receiving any treatment from your GP for the anaemia - like Iron ? Your Ferritin could be a little higher - around 90 I believe.... Do not worry about the cholesterol - it is fine. We need cholesterol and over 80% is produced in the body - so it is needed. The brain needs cholesterol and it is utilised for healing - VitD and so many other things. It is produced for a reason.

The thyroid is connected to over 300 conditions so it is important to try and get it right. I would still ask for the FT3 to be tested....and the anti-bodies I mentioned earlier. The tablet you take is T4 - a storage hormone - normally produced by a healthy thyroid. It does very little in the body. This has to convert into T3 in the liver/kidneys and elsewhere. T3 is needed in almost every one of the 30 trillion cells in our body - so when it is low you can imagine why things begin to go wrong. Hence it is VERY important to know what the blood result is.

It is a good idea to always ask for copies of all your blood tests so you can monitor your own progress - it is after all your right.

I have Hashimotos and low B12 and have learnt from others who know more than me !!

Everyone is different, of course, but my family's experience with B12 def, and autoimmune disease is that adequate B12 and thyroxine are both essential. Are you sure you having enough B12?

As your GP has already reduced thyroxine, it seems unlikely that reducing it even further would help - most people struggle to obtain optimum treatment in the first place! Some of your symptoms could also be exacerbated by the menopause,

Hashimoto's is the most common cause of thyroid disease and there is increasing evidence that avoiding gluten can help reduce antibodies.

I only saw doc couple of days ago so only just reduced from 125 to 100 daily. I have been on 150 for nearly 20 years since I was 22 and ive been ok but since I was diagnosed with PA 5 yrs ago, Vit D, iron, folic acid thyroid have gone crazy as a result I now take Vit D, and folic acid and I inject 2 ml of B12 per week as 1 ml every 3 months wasn't working for me, doc doesn't agree with me self injecting but I know how I feel. I could go on about all my symptoms but I have so many its boring! I think I am also going through menopause as getting really hot loads of digestive problems so what you said about gluten is interesting, my daughter has just bought me a book called 'Deliciously Ella' hoping to try some gluten free recipies.

I find the T4 T3 TSh thing really confusing not really sure about what to do next.

You need to get FT3 tested. After many years on Levo it isn't uncommon to lose the ability to convert the T4 to T3. I can recommend the site and book Stop the thyroid madness.
