Hello everybody can anyone advise?

I was taken off my Rucaparib after one course because my cholesterol soared and white platelets were shot. In parallel A scan showed my right kidney was swollen and barely functioning.

I had a nephrostomy last week and everything seemed good. I was warned there could be some discomfort and perhaps bit of blood in urine for a day or two but all was fine. Oncology told me to restart my paribs but on a lower dose.

Thursday I went up for follow up blood tests and they showed that my kidney function was even worse. Today I have ache in my left side kidney area and my bag is filling up with deep red urine. I am going up for a scan and more bloods to see what’s going on on Monday.

It could be my Parib or.....what? Has anyone else had experience surrounding Paribs and kidney problems?

Another concern is I’m finding it difficult to coordinate urology and oncology and GP. They wouldn’t win a synchronised swimming competition and it’s very stressful.

Dear Just,

This sounds like an awful and worrying state of affairs.

It seems as though there is not enough information out there about the devastating effects Rucaparib can have on your kidneys.

And, your team really needs to work with you on this.

Here is a bit of an abstract I found that might be helpful:

Recommendations: patients receiving rucaparib who develop an elevation of any grade in serum creatinine by definition and are suspected of having acute kidney injury (AKI) should first undergo the appropriate clinical assessments and investigations to exclude and treat other causes of AKI, such as dehydration or obstructive uropathy, which can occur in patients with advanced EOC who have a high burden of peritoneal or nodal disease. It may be appropriate to withhold rucaparib while these preliminary investigations are undertaken. Mild serum creatinine elevations do not require dose modification. Specifically, no dose adjustment is needed for patients with mild to moderate renal impairment (defined as CrCl, 30–89 mL/min; Table ​Table6).6). In patients who have CrCl <30 mL/min or require dialysis, dose recommendations have not been determined as no dedicated clinical studies of rucaparib have been conducted in these patients. We recommend monthly monitoring of renal function in patients taking rucaparib.

Thank you Laura that is indeed worrying. I will TRY and contact someone tomorrow I am going to the hospital for a scan perhaps I can get to see my CNS. hugs to you too.

I have had a couple of kidney scans- think it was to see if there was a blockage. I don’t know anything about side effects of Rucaparib.

Hi KBO, I’m sorry you’re having such disappointing results from Rucaparib. We had such high hopes didn’t we! I’ve been on Rucaparib for about 6 weeks now. My blood tests show kidneys are fine. Everything else seems ok except CA 125 has crept up from 9 to 14. Doesn’t seem very high but my CA has never been very high in spite of having 3 large tumours. My next blood tests are July 4th. No CT scan in sight. No idea if it’s working or not. I’ll let you know what happens. Niraparib didn’t suit me so perhaps if Rucaparib doesn’t suit you Niraparib might. You must be feeling very worried but hopefully you’ll find a solution next week. Laura has given you some good, helpful information which I’m going to read myself. Thinking about you .

Thanks Angela. I’m going to hospital for a scan this morning. I just hope I don’t end up with a double nephrostomy.
