So I got my mri results back today. I was negative for hla b27 the mri showed minor wear and tear of all of my spine and inflammation in my pelvis sacroiliac joints as well as the auto immune inflammatory peripheral athritis. Osteoarthritis of both hips too. I'm a bit confused but they are keeping me on 25mg of methotrexate and starting me on amgevita

Sounds like you have a seronegative spondyloarthritis, which can affect both the spine and sacroiliac (axial) and peripheral joints. It is a relative of ankylosing spondylitis and still under the umbrella of autoimmune arthritis. At the end of the day, a specific diagnosis or label for your inflammatory arthritis is not necessary to continue treatment, and many of us have had our diseases morph from one diagnosis to another, or sometimes with overlap. Good to hear that you are being treated, as Rheumatologist can vary in their degree of confidence in treating seronegative diseases, often to the detriment and dismay of patients who are not offered biologics. Hope you progress well.

Hi bakerannie yeah my last letter said seronegative peripheral and spine auto immune inflammatory athritis linked to my pancolitis which I was diagnosed with 2 years ago. I was googling and seen ankylosing spondylitis usually starts in the sacroiliac joints so I was wondering if this was possible early AS I'm struggling with my mobility just now I'm just praying things improve 🙏🙏

As the two are so closely related, it is sometimes difficult to differentiate between them. Having reactive autoimmune arthritis due to your colitis leans you more towards the spondyloarthritis, however treatment is similar for both regardless. I do hope you start feeling better soon.

I didn’t realize that the two look are closely related. Thanks for the insight. My arthritis sounds very much like the OP’s arthritis and came on fast and furious after a bowel perforation. I’m seronegative too. I’ve been on and off biologics with limited success.

Hi tthiele I'm the same as you it has all hit me quickly I've been on methotrexate since September for the peripheral joints but it's made no difference just hoping the amgevita helps with the back because its awful. What biologics are you on

hi Brian,

I was on Enbrel and Humira. unfortunately I am allergic to TNF inhibitors (found that out from the way my skin reacts to them and getting Lupus as a side-effect). (Lupus went away after stopping medication.) Not taking anything right now but have Skyrizi coming. I was not given MTX because I got severe liver inflammation from the bowel infections and MTX can affect the liver.
